Meta-Analysis And Mechanism Study On Treatment Of Chronic Heart Failure With Linggui Zhugan Decoction

PurposeTo systematically evaluate the clinical efficacy of Lingguizhu gan Decoction and its addition and reduction on chronic heart f ailure and explore the relevant mechanismMethodsMeta-analysis was conducted by using computer to search sev en databases including CNKI,Wanfang Database,VIP database,China Biomedical Literature Database,Pubmed,Embase and Coc hraneLibrary with corresponding retrieval strategies.Relevant lit erature information was recorded after literature retrieval by sea rching relevant search terms.The literature that met the researc h criteria was meticulously screened based on the predeterminedBased on network pharmacology,the search of potential tar gets and related compounds of Lingguizhugan Decoction was co mpleted,and related to the target of CHF,the intersection of th e two was made to determine the regulatory network of Linggui zhugan Decoction-target-chronic heart failure,and then the visualization was implemented.The target,protein interaction ne twork construction,GO enrichment and KEGG enrichment map w ere analyzed and drawn by relevant software and package.Results1.A total of 2 0 randomized controlled trials were included in t his meta-analysis,and the results showed that the efficacy of L ingguizhu Gan Tang plus or minus combined with conventional Western medicine was superior to that of the control group und er conventional Western medicine.For example,the clinical effi cacy and TCM syndrome scores were more significantly improve d,6MWP was also significantly increased,NT-proBNP was signi ficantly reduced,LVEF and other cardiac ultrasound indicators were also more significantly improved,the difference was statist ically significant(P<0.05);2.75 regulatory targets and 350 targets highly correlated with chronic heart failure were screened out.There were 31 related active compounds,including β-sitosterol,Kosophorin,iviogenin,glycyrrhizin and so on.106 regulatory targets were screened by PPI network construction,and 2 0 target proteins with core roles were obtained,including MAPK1,NOS3,STAT3,NOS2,AKT1,etc.Conclusions1.It has been confirmed that Linggui Zhugan Decoction can treat chronic heart failure after addition or reduction,and its addition or reduction formula combined with conventional Western medicine is significantly superior to the treatment of Western medicine alone.2.The mechanism of action of Lingguizhugan Decoction was initially explored by means of network pharmacology,and it was found that the reason why Lingguizhugan Decoction could treat chronic heart failure was the result of the combined action of many aspects,which confirmed that TCM treatment is the synergistic effect of multicomponents,multi-targets and multi-pathway s,indicating the direction for the development of related basic research in the next step.
