Observation On The Curative Effect Of Jiawei Zishui Qinggan Decoction In Treating Elderly Patients With Isolated Systolic Hypertension(Liver And Kidney Yin Deficiency Type) And Early Renal Damage

Objects:Through the study of the intervention effect of Jiawei Zishui Qinggan Decoction on elderly patients with Isolated Systolic Hypertension（liver and kidney Yin deficiency type）,the clinical effect of this prescription on elderly patients with Isolated Systolic Hypertension with liver and kidney Yin deficiency type and early renal damage was discussed.Methods:This study included 68 elderly patients with Isolated Systolic Hypertension（liver and kidney Yin deficiency type）and early renal damage who met diagnostic criteria and were hospitalized in the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from December2021 to December 2022.After inclusion,34 cases were randomly divided into control group and treatment group,and 4 cases fell off during treatment（3cases in control group and 1 case in treatment group）.In the course of treatment,on the basis of conventional treatment,the control group was orally given Sacubitril Valsartan Sodium Tablets,and the treatment group was orally given Jiawei Zishui Qinggan Decoction,a total of 2 weeks of treatment,a total of 1 course.The TCM syndrome scores,24-hour ambulatory blood pressure and early renal damage indexes before and after treatment were recorded and compared between the two groups.Meanwhile,the safety indexes and adverse reactions were also recorded and evaluated.Results:1.Comparison of TCM syndrome scores:The total TCM syndrome scores and total scores of major and minor symptoms of the two groups were compared before and after,and the results showed that the total scores of TCM syndrome,major and minor symptoms decreased significantly,and the decrease was more significant in the treatment group than in the control group,with statistical significance（P<0.05）.In terms of individual scores,there were statistically significant differences between the two groups in vertigo,five upset heat,dry mouth and throat,tinnitus,insomnia and other aspects before and after treatment（P<0.05）,while there were no significant differences between the control group and the treatment group in the aspects of waist and knee tenderness,deafness,hot flusher and night sweats,body wasting,forgetfulness and so on（P>0.05）.2.Comparison of TCM efficacy:the total effective rate of the treatment group was 90.91%,and the total effective rate of the control group was83.87%,the difference was statistically significant（P<0.05）.3.Comparison of blood pressure levels:before and after treatment,the treatment group and the control group decreased in systolic blood pressure levels and the treatment group was better than the control group,the difference was statistically significant（P<0.05）.4.Comparison of early renal damage indicators:Cystatin C and bloodβ₂microglobulin indicators,which represent indicators of early kidney damage,were compared between the two groups before and after treatment,and the indicators of both groups were decreased,and the treatment group was better than the control group,the difference was statistically significant（P<0.05）.Conclusions:1.Jiawei Zishui Qinggan Decoction can reduce the TCM syndrome score of elderly patients with isolated systolic hypertension（liver and kidney yin deficiency type）and improve the clinical symptoms of patients.2.Jiawei Zishui Qinggan Decoction can effectively regulate the blood pressure level of elderly patients with isolated systolic hypertension（liver and kidney yin deficiency type）,and has a positive effect on delaying the process of renal damage.
