Objective: By exploring the clinical efficacy and application value of 3D printing personalized osteotomy guide plate assisted TKA based on AIKNEE preoperative planning system,we compared its advantages and disadvantages with conventional TKA.Methods: A retrospective study was conducted on 22 patients(24 knee replacements)with primary knee osteoarthritis who met the inclusion and exclusion criteria and were scheduled to undergo TKA surgery in our hospital from January 2021 to September 2021.Among them,12 patients underwent AIKNEE preoperative planning and printed personalized osteotomy guide plates,while 12 patients were in the conventional surgery group.By further comparing the baseline data,prosthesis type,surgical time,blood loss,HKA deviation value,FFC deviation value,FTC deviation value,LTC deviation value,VAS score,KSS score,and postoperative complications between the two groups.Results:1.Compared with the conventional group,the operation time and intraoperative bleeding in the planning group were reduced,and the difference between the two groups was statistically significant(P<0.05).2.The complete coincidence rate of preoperative planning model and intraoperative practical application model of the planning group was high(83.3%),which has surgical reference significance;3.KSS score in the planning group was better than that in the conventional group at 3 days,1 month and 6 months after surgery(P<0.05);4.There was no significant difference in the final postoperative VAS pain score between the two groups(P>0.05).5.The HKA deviation,FFC deviation,FTC deviation and LTC deviation of the planning group were all better than those of the conventional group,and the difference between the two groups was statistically significant(P<0.05).Conclusion:(1)Implement preoperative planning of AIKNEE,accurately install prostheses,achieve precise treatment,and improve the accuracy of TKA;(2)3D printing personalized osteotomy guide can improve the accuracy of intraoperative osteotomy,easy operation,and better postoperative function;(3)Shorten the operation time,less postoperative complications,improve the safety of surgery;(4)Compared with conventional TKA,the same prosthetic size can improve patients’ satisfaction after surgery.Through the AIKNEE preoperative planning combined with 3D printing osteotomy guide,the surgical process is simplified,the operation time is reduced,and the surgical complications are also reduced.The short-term postoperative treatment effect is satisfactory,in line with the concept of rapid recovery and the needs of patients,which provides a new scheme for the realization of accurate TKA surgery. |
