[Objective]:To compare the preoperative planning with the two-dimensional digital template(TDTD),and investigate the accuracy and effectiveness of Artificial intelligence system(AIS)in the clinical application of prosthesis implantation in primary total hip arthroplasty.[Methods]: From June 2020 to December 2022,a prospective study was conducted on 100 patients who needed primary total hip replacement due to hip joint disease,and patients who met the inclusion criteria were randomly assigned to the AI group.and 2D group,50 cases in each group,among which AIHIP system was used for preoperative planning in AI group,and Smart Joint software was used for preoperative planning in 2D group.According to the condition,the operation time,intraoperative blood loss,preoperative and postoperative Harris hip score,preoperative and postoperative lower limb length difference,acetabular prosthesis abduction angle,femoral stem-medullary cavity axis angle were compared between the two groups and femoral stem-canal ratio.[Results]: All patients were followed up for 6 months,and the incisions healed by first intention.There was no significant difference in general data such as surgical site,age,gender,body mass index(BMI)and Harris hip score between the two groups(P>0.05).The imaging measurement results showed that there was no statistical significance in the acetabular prosthesis abduction angle between the two groups after operation(P>0.05),and there was no statistical significance in the postoperative femoral stem-medullary cavity axis angle(P>0.05).The ratio of femoral stem to medullary cavity was statistically significant(P<0.05).There was a significant difference in the length of the lower limbs between the AI group and the 2D group before and after operation,both of which were statistically significant(P<0.05).The accuracy rates of acetabular prosthesis and femoral prosthesis in AI group were 76%(38/50)and 70%(35/50),respectively,and the accuracy rates of acetabular prosthesis and femoral prosthesis in 2D group were 58%(29/50)and 54%(27/50),the accuracy rate of the acetabular prosthesis and femoral prosthesis within one size was 96%(48/50)and 96%(48/50)in the AI group,The 2D group was 76%(38/50)and 70%(35/50)respectively,and the difference in the accuracy rate of the prosthesis between the two groups was statistically significant(P<0.05).The average operation time and average blood loss in the AI group were less than those in the 2D group,both of which were statistically significant(P<0.05).In addition,there was statistical significance in the Harris hip score between the AI ??group and the 2D group at 1month and 6 months after operation(P<0.05).[Conclusion]:The AIHIP system can achieve precise planning for THA surgery.Compared with the two-dimensional digital template,it has higher accuracy and effectiveness.It can assist the operator to shorten the operation time,reduce intraoperative blood loss,and promote The patient’s hip joint function recovered postoperatively. |
