Objectives: The purpose of this study is to understand the current status of stroke patients with broca aphasia,explore and analyze the impact of the integrated management model of medical care and patient on speech function,depression level,and quality of life of stroke patients with broca aphasia,explore the feasibility and effectiveness of clinical application of this model,and provide reference and basis for subsequent clinical research on stroke broca aphasia.Methods: A total of 110 patients with broca aphasia after stroke hospitalized in the rehabilitation department of a first-class hospital of Xinjiang Medical University from July 2021 to July 2022 were selected as the research objects.Based on the different intervention programs received by the subjects,they were divided into control group and observation group,there were 55 patients in each group.The treatment plan of the two groups was the same,and the control groupreceived the routine management mode of rehabilitation departmentt,and the observation groupwas given the integrated management mode of doctors,nurses and patients.The language function,aphasia degree,depression degreeand quality of life of the two groups were evaluated before and after the intervention.To analyze the effects of two management models on language function and qualityof life in stroke patients with broca aphasia.Results:1)The baseline data showed that there were no significant differences in demographic data,disease data,language function,depression degree,and quality of life between the two groups(P>0.05),which were comparable.2)Univariateand multivariate linear regression analysis showed that education level,annual household income,aphasia degree,speech function and depression degree have a certain impact on the quality of life of stroke patients with broca aphasia,and the scores of quality of life are significantly different(P<0.05).3)Before intervention,the total score of speech function in the control group was(252.53±17.35)points,and that in the observation group was(255.18±20.80)points.The speech function of the two groups was at a low level,and there was no statistically significant difference in the speech function score between the two groups(P>0.05).Three months after intervention,the total score of speech function in the controlgroup was(319.15±12.43),and that in the observation group was(366.47±14.14).There werestatistically significant differences in the scores of spontaneous conversation fluency,spontaneous conversation information,repetition,naming,reading,listening comprehension,writing and total score between the two groups(P<0.001).4)Before the intervention,the score of aphasia depression in the control group was(25.25±2.35),and the score of aphasia depression in the observation group was(24.51±2.40).The degree of depression in the two groups was more serious,and there was no statistically significant difference in the score of depression between the two groups(P>0.05).Three months after intervention,the score of aphasia depression in the control group was(21.71±2.17),and the score of aphasia depression in the observation group was(18.42±2.87).The score of depression degree of the two groups was statistically significant(P<0.001).5)The total score ofquality of life was(378.93±26.77)in the control group and(377.67±27.04)in the observation group.Before the intervention,the quality of life of the two groups was lower than the national norm,and there was no statistically significant difference in the quality of life between the two groups(P>0.05).Three months after intervention,the total score of quality of life was(426.18±16.19)in the control group and(474.35±27.49)in the observation group.There were statistically significant differences in the scores of general health status,role-physical,vitality,role-physical,mental health,role-emotional,bodily pain,social function and total score between the two groups(P<0.001).Conclusion:The language functionand quality of life of stroke patients with broca aphasia are generally at a low level,and the incidence of aphasia depression is high and severe.The integrated management model of doctors,nurses and patients can effectively improve the speech function of patients,reducethe incidence of aphasia depression,and finally,the quality of life of patients canbe improved.
