Objective: To analyze the clinical effect of image-guided interventional puncture technique in the treatment of pelvic abscess.Methods: A total of 298 cases of pelvic abscess were collected from the Department of Obstetrics and Gynecology of Shengjing Hospital affiliated to China Medical University from January 2011 to February 2021.They were divided into "interventional puncture group" and "surgical treatment group" according to the treatment method.The clinical data of the two groups of patients were analyzed,and the patients who retained fertility function were followed up.Results:(1)There was no statistical difference in the basic data between the two groups.The preoperative temperature of patients in the puncture group was slightly higher than that in the operation group(P=0.017);The white blood cell count(P=0.008),neutrophil percentage(P<0.001)and CRP(P=0.005)in the puncture group were higher than those in the operation group;The median diameter of abscess in the puncture group was greater than that in the operation group(P=0.001).(2)The hospitalization time(P<0.001),postoperative fasting time(P<0.001),the proportion of patients transferred to ICU after operation(P=0.004),the time of antibiotic application after operation(P<0.001)and the total hospitalization cost of the patients(P<0.001)in the puncture group were lower than those in the operation group,and the difference was statistically significant;The median drainage time of patients in the puncture group was higher than that in the operation group,and the difference was statistically significant(P<0.001);There was no significant difference between the two groups in the length of ICU stay and the number of postoperative complications.From the perspective of laboratory indicators,the percentage of neutrophils(P=0.001),CRP(P=0.013)and HGB(P<0.001)recovered to normal after operation in the puncture group was higher than that in the operation group,and the difference was statistically significant.The decrease of hemoglobin in the puncture group was lower than that in the operation group after operation,and the difference was statistically significant(P<0.001).(3)There was no statistical difference in the number of recurrent cases,postoperative symptoms,menstrual changes and menopausal symptoms between the two groups(P>0.05).(4)The preoperative hospital stay,the number of abscess,the amount of fluid extracted,the bacterial culture,whether the catheter is inserted,the length of catheter insertion,the length of antibiotic use,the local antibiotic irrigation,the puncture path,and the imaging method in the puncture group were not related to the prognosis of patients,and the difference was not statistically significant(P>0.05).Conclusion: The interventional puncture technique guided by imaging is superior to surgical treatment in clinical efficacy and economy in the treatment of pelvic abscess,which can shorten the clinical treatment time and improve the quality of life of patients.In clinical practice,we should fully combine the patient’s condition and choose individualized puncture treatment plan.
