A Study On The Curative Effect Of Pus Puncture And Cotton Pad Binding In The Treatment Of Plasma Cell Mastitis In The Abscess Period

Background:Plasma cell mastitis,also known as mammary duct ectasia,periductal mastitis,so far has a history of near a hundred years since first reported in 1920s.It’s a chronic breast benign disease based on duct ectasia and plasmocyte infiltration.It often attacks in nonlactation period.Non-cyclical breast pain,nipple discharge,lump below mammary areola,abscess beside mammary areola and the formation of fistula are its main clinical symptoms.In traditional Chinese medicine,we named it comedo mastitis.Caused by non-bacterial inflammatory,Plasma cell mastitis is a self-allergic inflammation.There is no specific treatment in western medicine.Usually,the treatment methods are antibiotic therapy,hormone therapy and surgery which are respond poorly.According to different pathological process,it can be divided to four phases:nipple discharge phase,lump phase,abscess phase and fistula phase.Among the four phases,the abscess phase is the most important to cure and the thorniest.If being treated properly,the abscess will get controlled even healed.And then the disease cured.If the abscess and mastitis begin to spread because of mistreatment,as one falls another rises,and multiple fistula,sinus and fester will result in difficult in the healing process and making the patients suffering.In this case,the disease will create scars and transformation in breast,and make harm to both physical and mental health of the patients.At present,the regular treatment of plasma cell mastitis at abscess phase is abscess incision drainage and change dressing after surgery.The shortcomings of it includes heavy local damage,incision is difficult to union and long course of the treatment,complicating pain coursed by fistula,sinus and surgical incision.Openness wound brings cross infection.It will also have bad influence on the appearance of breast and bring psychological trauma.And the healing process brings about financial burden.So it becomes a Gordian knot that to cure plasma cell mastitis at abscess phase.Abscess puncture aspiration and pad pressure method is a characteristic method,integrated traditional and Western medicine,to healing the abscess of breast.With the increase number of patient of plasma cell mastitis,it has been performed more and more and proved to have a better clinical effect.Comparing with the abscess incision drainage,it possesses lower expenditure,more effective,less pain,smaller damage,basically no scar,lower fistula occurrence rate.And it is easy and simple to handle.Objective:Take the patients with a plasma cell mastitis at abscess phase as the object of study,and take the incision drainage with regular dressing change as the contrast therapies.Observing the clinical effects of abscess puncture aspiration and pad pressure method.Discuss the advantages of abscess puncture aspiration and pad pressure method in the aspect of cure rate,abscess improved conditions,pain degree,breast appearance damage and health economic.Methods:A total of 34 plasma cell mastitis at abscess phase patients were selected randomly,according to the proportion of 1:1,divided into test group and control group.The 17 patients in test group had been treated by abscess puncture aspiration and pad pressure method.And 17 patients in control group had been treated by incision and drainage and regular dressing.The groups would all take same Chinese medicine decoction—Ruyongfang.The treatment would last for two months.Observing the indicators of cure rate,abscess improved conditions,pain degree,breast appearance damage and health economic.Results:34 patients were initially inducted into this study,while subsequently 2 cases withdrew from the study,there were 15 patients in the test group and 17 patients in the control group.Compare the abscess improved of two groups,when it comes to cure rate,shrinking the size of lump in palpation,reducing the breast abscess under the ultrasonic imaging the,improving the appearance of breast,reducing frequency of dressing change and lower therapy cost,the difference was statistically significant(P<0.05).But in the aspect of shorten the duration of pain,reducing pain VAS score,reducing the healing process and recurrence rate,two groups has similar results(P>0.05).Conclusion:Abscess puncture aspiration and pad pressure method are proved effective in reducing the size of breast abscess,release the breast pain,maintain breast appearance.It can improve the quality of life of the patients by means of reducing pain for patients,reducing scarring,reducing the frequency of dressing change,reducing the treatment cost.The method is reliable and easy to handle.It meets the requirement of clinic and can be generalized in the medical institutions.
