Objective: To investigate in the peripheral blood the express level relationship between the expression of soluble CD14(SCD14),Toll-like receptor 4(TLR4)and the stability of coronary atherosclerotic plaque in patients with coronary heart disease(CHD).Methods: From October 2018 to February 2020,146 patients with coronary heart disease(CHD group)in cardiovascular department of our hospital were selected continuously,who underwent coronary angiography and agreed to participate in the study.Coronary heart disease patients included 38 patients with stable angina pectoris(SAP),55 patients with unstable angina pectoris(UAP)and 53 patients with acute myocardial infarction(AMI);At the same time,146 patients with coronary atherosclerosis were included in the study as the control group.The severity of coronary artery disease in patients with coronary heart disease was assessed by Gensini score.Coronary artery CT imaging was used to classify the coronary atherosclerotic plaque,and coronary atherosclerotic plaque was classified into soft plaque,mixed plaque and calcified plaque.The levels of TLR4 in peripheral blood and SCD14 in serum were measured by Flow cytometry and enzyme-linked immunosorbent assay(Elisa),respectively.The correlation between the expression levels of soluble CD14 and TLR4 and the Gensini score was analyzed by Pearson’s method.Binary Logistic regression was used to analyze the influencing factors of coronary atherosclerotic plaque instability in patients with coronary heart disease.Results: Compared with those in the control group,the expression levels of sCD14 and TLR4 in SAP group,AMI group and UAP group were higher(P < 0.05),compared with those in SAP group,the Gensini score,sCD14 and TLR4 expression levels in peripheral blood in AMI group and UAP group were higher(P < 0.05),compared with that in UAP group,Gensini score of AMI group was higher(P < 0.05).Compared with those in the hard plaque group,the expression levels of sCD14 and TLR4 in the soft plaque group and mixed plaque group were higher(P < 0.05),compared with those in the mixed plaque group,the expression levels of sCD14 and TLR4 in the soft plaque group were higher(P < 0.05).The expression levels of sCD14 and TLR4 were positively correlated with Gensini score(P < 0.05).Logistic regression analysis showed that high level of TLR4,high level of sCD14 and high Gensini score were the risk factors of unstable atherosclerotic plaque in patients with coronary heart disease(P <0.05).Conclusions: The expression level of TLR4 in peripheral blood and the level of sCD14 in serum of patients with coronary heart disease are significantly increased,they are closely related to the instability of coronary atherosclerotic plaque,and there is a significant correlation between TLR4,sCD14 and the severity of coronary artery disease.
