BackgroundAs the leading cause of death worldwide,cardiovascular disease has a high rate of disability and mortality.In recent years,the prevalence,morbidity and mortality of cardiovascular diseases in Chinese residents are on the rise.With respect to current background knowledge,atherosclerosis(AS)is associated with a number of factors,including vascular endothelial cell injury and dysfunction,abnormal blood lipid and inflammatory factors,etc.Clinically,it presents chest pain,chest tightness and other symptoms,with or without hemodynamic abnormalities.Under the continuous influence of various risk factors,the degree of vascular stenosis becomes more serious,and the disease course is more advanced.Chronic total occlusion(CTO)is formed,which poses certain challenges to the current percutaneous interventional therapy.MicroRNA-34a(miR-34a)is a class of small molecular RNA that has been shown to be associated with a variety of cardiovascular diseases in recent years,including myocardial ischemia,ventricular remodeling and hypertension.Relevant studies have shown that miR-34a regulates the occurrence and development of atherosclerosis through multiple pathways,and may play an important role in coronary heart disease based on coronary atherosclerosis.Purpose1.Plasma miR-34a levels of CHD patients and non-CHD patients were compared.2.The correlation between plasma miR-34a level and the degree of coronary artery disease in patients with coronary heart disease was analyzed.3.To explore whether plasma miR-34a can be used as a new biomarker to evaluate the degree of coronary artery lesions in patients with coronary heart disease.Objective and methods93 inpatients in the department of cardiovascular medicine of the second people's hospital of Guangdong province from April 2018 to December 2018 were selected.After admission,general data of the selected patients were improved and venous blood samples were collected.Blood samples were centrifuged and the plasma total microRNA content was extracted.The plasma miR-34a content was detected by reverse transcriptional fluorescence quantitative PCR.(1)Coronary artery interventional imaging examination,according to the result of check group clear coronary atherosclerosis heart disease(CHD)patients and included in the experimental group of 73 people,which can be divided into:1.The coronary artery chronic complete occlusion(CTO)group of 18 people;(2)The coronary artery chronic complete occlusion(CTO)group 55 people;Twenty patients in the negative group were included in the control group,and the plasma miR-34a levels of the control group and the experimental group were compared,and the plasma miR-34a levels of the CTO group and the non-CTO group were compared.(2)The experimental group was given SYNTAX score to evaluate,which was divided into SYNTAX low score group(0-22 points),middle score group(23-32 points)and high score group(33 points)according to the hierarchy of score,and plasma miR-34a levels in each subgroup were compared.(3)The Gensini score was divided into G1(0-32 points),G2(32-68 points),G3(68-110 points)and G4 group(110-256 points)from low to high according to the score,and plasma miR-34a levels iin each subgroup were compared.(4)According to SYNTAX score and Gensini score,statistical analysis was applied to analyze whether there was statistical significance between plasma miR-34a level and the two scoring systems.Results(1)Plasma miR-34a level was higher in the experimental group than in the control group(P<0.05);The expression level of CTO group was higher than that of non-CTO group(P<0.05).In the SYNTAX scoring system,as the score increases,the expression level of miR-34a increases in gradient among the low group,the middle group and the high group.There are differences between each group(P<0.05).(3)Within the Gensini scoring system,the expression level of mir-34a between the G1,G2,G3 and G4 groups increased in gradient with the increase of integral,and there were differences between each group(P<0.05).Conclusion(1)Compared with non-chd patients,plasma miR-34a level was higher in CHD patients;Among all the CHD patients,plasma miR-34a level of CTO patients was higher than that of non-CTO patients.(2)With the aggravation of coronary artery disease,plasma miR-34a expression level gradually increased,and the two were positively correlated.(3)Plasma miR-34a level can be used as an indicator to evaluate the degree of coronary artery disease.
