| Objective: Based on the "Occupational Disease and Occupational Health Information Monitoring System" and its relevant data in Liaoning Province,to statistically describe the incidence and distribution characteristics of occupational pneumoconiosis in Liaoning Province,analyze the burden of occupational pneumoconiosis in Liaoning Province,and evaluate the advantages and disadvantages of the past occupational pneumoconiosis prevention and control work,so as to provide evidence for the future development of pneumoconiosis prevention and control methods and countermeasures in Liaoning Province.Research methods: The basic data(age,sex,age of onset,reporting time),occupational status(disease type,type of work,dust exposure duration,stage of occupational pneumoconiosis,location,type of enterprise and scale)of 11903 patients with occupational pneumoconiosis diagnosed by Liaoning Provincial Occupational Disease Diagnosis Institution and reported to the "Occupational Disease and Occupational Health Monitoring System" from January 1,2011 to December 31,2020 were collected.The above 11903 subjects were divided into three grades according to the stages of occupational pneumoconiosis: Phase I,Phase II and Phase Ⅲ,and 595 patients with occupational pneumoconiosis were selected as the subjects by stratified random sampling.The overall value and average of the years live with disability(YLDs)of patients with occupational pneumoconiosis at different classification levels were calculated by using data such as age,length of service exposed to dust,stage of occupational pneumoconiosis,and disability grade.Results: This study collected 11903 cases of occupational pneumoconiosis reported in Liaoning Province from 2011 to 2020,including 12 types of silicosis,coal worker’s pneumoconiosis and welder’s pneumoconiosis.Silicosis is the most common disease,followed by coal worker’s pneumoconiosis.In the stage of occupational pneumoconiosis,Phase Ⅱ was the most,followed by Phase Ⅰ and Phase Ⅲ.The reported cases of occupational pneumoconiosis in Liaoning Province showed an overall trend of "rising first and then falling",with two peaks in 2014 and 2017,and a sharp decline after 2017. Occupational pneumoconiosis accounted for 91.84% of the occupational disease cases in Liaoning Province.The average length of service of occupational pneumoconiosis cases reported in Liaoning Province was 15.78 years.Cases with less than 5 years of dust exposure accounted for 4.95%,mainly concentrated in 2012,mainly in Huludao City,and concentrated in the mining industry.In terms of economic types of enterprises,occupational pneumoconiosis in Liaoning Province is mainly distributed in private economic enterprises.In terms of enterprise scale,occupational pneumoconiosis cases in Liaoning Province are mainly distributed in small enterprises.The total value of YLDs of595 occupational pneumoconiosis patients selected in the disease burden study was7152.76 person-years,with an average of 12.02 person-years.It can be estimated that the total disease burden of 11903 patients is 143091.26 person-years,with an average of12.02 person-years.From the perspective of disease type,the overall YLDs of silicosis in595 patients were significantly higher than that of coal workers’ pneumoconiosis.In terms of stage,the total value and mean value of YLDs in patients with stage Ⅲ pneumoconiosis were higher than those in stage Ⅱ and stage Ⅰ.From the perspective of dust exposure,YLDs of patients with dust exposure of 11-20 years are the highest,followed by dust exposure of ≤ 10 years.From the perspective of enterprise economic type and enterprise scale,the privately owned enterprises YLDs are the highest.From the perspective of enterprise size,the average YLDs of microenterprises are the highest.Conclusion: From the epidemiological characteristics,the total number of occupational pneumoconiosis patients reported in Liaoning Province from 2011 to 2020 is large.Phase Ⅱ and Ⅲ patients with occupational pneumoconiosis account for a large proportion,and the disease is relatively serious.From the perspective of disease burden analysis,the total burden of occupational pneumoconiosis in Liaoning Province from 2011 to 2020 was higher than the national average.Occupational pneumoconiosis causes high life loss due to disability.Liaoning Province should take the prevention and treatment of occupational pneumoconiosis as the focus of occupational disease prevention and treatment,and pay attention to the prevention,early diagnosis and prognosis of the disease. |
