Based On Data Mining,Analyze The Medication Rule And Academic Experience Of Ruan Shiwei In Treatment Of CKD With Syndrome Of Qi Deficiency Of Lung And Kidney

Objective:To analyze the medication rule of Professor Ruan Shiwei in treating chronic kidney disease with syndrome of qi deficiency of lung and kidney through data mining technology,summarize his clinical experience,medication characteristics and academic thoughts,and provide a reference for clinical practice.Methods:This study mainly collected medical records of patients in Professor Ruan Shiwei’s outpatient clinic from January 1,2018 to January 1,2022 Western medicine met the diagnostic criteria of chronic kidney disease,while Chinese medicine met the syndrome of qi deficiency of lung and kidney.After standardized processing of patients’ general information,major complaints,symptoms,test results,drug prescriptions and other relevant information,data were input into the "Traditional Chinese Medicine Inheritance Assistance Platform(V2.5)" system software for analysis to mine the rules of medication,and summarize academic experience of Professor Ruan Shiwei.Results:1.A total of 127 patients with chronic kidney disease with syndrome of qi deficiency of lung and kidney and 302 prescriptions were included in this study.The average age of patients is 49 years old and the age group is mainly between 40 and 59 years old.The majority of them are women,with a male to female ratio of 1:1.35.Among them,57 patients with CKD1 stage accounted for the highest proportion is about 44.9%..The primary disease type was chronic glomerulonephritis in 36 cases,followed by Ig A nephropathy in 27 cases and nephrotic syndrome in 22 cases.2.According to the frequency statistics of 302 prescriptions,we found that Professor Ruan Shiwei used Licorice,Astragalus mongholicus,Ophiopogon,Radix pseudostellariae,Serissa serissoides,Poria cocos,Plantain seed,Chinese yam,Plantain,Chinese angelica,Lotus fruit and other traditional Chinese medicines in the treatment of chronic kidney disease with syndrome of qi deficiency of lung and kidney.The four characteristics of traditional Chinese medicine are cold,warm and smooth.The five tastes of traditional Chinese medicine are mainly sweet,bitter and acrid.The main Meridian tropism of traditional Chinese medicine are those of lung,spleen,liver,kidney and stomach.3.Using the function of association analysis,a total of 52 groups of drug patterns were obtained,including "Astragalus membranaceus,Poria cocos","Radix Bupleuri,Astragalus membranaceus","Radix pseudostellariae,Lotus fruit",etc.Using the analysis function of the new formula,24 new drug combinations were obtained,including "Schizonepeta,Arctium Fruit,Platycodon grandiflorum","radix polygonati officinalis,adenophora tetraphylla,trichosanthes root","the dried rhizome of rehmannia,fruit of medicinal cornel,moutan bark,Chinese yam" and so on.According to the statistics of entropy clustering method,12 potential combinations of new formula were obtained,including "Prepared soybean,Arctium Fruit,mint,Tetrapanax papyriferus,Loquat leaf,Pardanthus","the dried rhizome of rehmannia,fruit of medicinal cornel,moutan bark,Chinese yam,dendrobium,lophatherum gracile,lotus leaf",etc.Conclusion:1.Professor Ruan Shiwei used Licorice,Astragalus mongholicus,Ophiopogon,Radix pseudostellariae,Serissa serissoides and other traditional Chinese medicines in the treatment of chronic kidney disease with syndrome of qi deficiency of lung and kidney.Qi-invigorating herbs,diuretic dampness excreting drugs,heat-clearing drugs,drugs for relieving exterior syndrome,medicines for relieving cough and reducing sputum were also part of the significant medication selection.2.In terms of prescription application,Professor Ruan mostly uses Dihuang Decoction,Qingxin Lianzi Drink,Qingshu Yiqi Decoction,Shengyang Yiwei Decoction,Shangjiao Xuanbi Decoction,Qiaohe Decoction,and other core prescriptions,and adds or subtracts them based on concurrent symptoms.3.In terms of treatment,based on the relevant theory of "mutual growth of gold and water",Professor Ruan dialectical applies theory of the healthy tendency of the organism and the sick tendency,to judge the location of the disease and the advance and fall of the healthy tendency of the organism and the sick tendency,was comprehensively adopted.Treatment focused on taking care of both tip and root,raising the healthy tendency of the organism and eliminating the sick tendency.He conducts comprehensive analysis and dialectical treatment.
