Objective By comparing the clinical efficacy of traditional Chinese medicine fumigation and washing combined with intradermal needling with simple intradermal needling in the treatment of windcold and dampness type of scapulohumeral periarthritis, this paper aims to find a better treatment plan for it and provide a certain reference value for its clinical treatment.MethodsSixty patients of 40 to 65 years old with wind-cold and dampness type of scapulohumeral periarthritis, who were inpatients and outpatients from the Department of Orthopedics and Traumatology,Zhangzhou Hospital of Traditional Chinese Medicine, affiliated to Fujian University of Traditional Chinese Medicine, were selected as the subjects of this study. They were randomly divided into an experimental group and a control group using a random number table method, with 30 patients in each group. The control group was treated by simple intradermal needling therapy, with the needle remaining for 24 hours. The next treatment was performed the second day after withdrawing the needle, with 10 times for a course of treatment, and a total of 1 course. The experimental group was treated with traditional Chinese medicine fumigation and washing combined with intradermal needling for 30 minutes, once every other day,10 times for a course of treatment, and a total of 1 course. After 30 minutes of fumigation and washing, the intradermal needling treatment was performed, with the same procedures and course of treatment as the control group. After the above treatment was completed, the two groups made functional exercises under the guidance of a physician, twice a day, once in the morning and afternoon, for 20 minutes each time, lasting until the day after the last treatment.Quantitative indicators such as VAS pain score,Constant-Murley shoulder function score, and TCM symptom score were recorded and analyzed before and 1 day after treatment in the two groups.SPSS22.0 was used for data analysis.Result1.Pre-treatment evaluation: There was no statistically significant difference between the two groups in gender, age, course of disease, distribution of affected limbs,VAS pain score,CMS score, and TCM symptom score of shoulder joint(P>0.05),which was comparable.2.VAS score: After treatment, the VAS score of the two groups of patients decreased compared to that before treatment, with a statistically significant difference(P<0.05);for inter group comparison, the score of the test group was lower than that of the control group, with a statistically significant difference(P<0.05).3.CMS score: After treatment, the shoulder pain,ADL,ROM scores in the CMS scores of the two groups of patients increased compared to those before treatment, with a statistically significant difference(P<0.05);for inter group comparison, the score of the test group was higher than that of the control group, with a statistically significant difference(P<0.05).4.TCM symptom score: The TCM symptom score of the two groups of patients decreased compared to that before treatment, with a statistically significant difference(P<0.05);for inter group comparison, the score of the test group was lower than that of the control group, with a statistically significant difference(P<0.05).5.Clinical efficacy evaluation results:(1)Test group: 9 cases were clinically cured,10 cases were significantly effective,7 cases were effective, and 4 cases were ineffective, with a total effective rate of 86.67%;(2)control group: 3 cases were clinically cured,9 cases were significantly effective,11 cases were effective, and 7 cases were ineffective, with a total effective rate of 76.67%.After treatment, the total efficacy of the two groups was compared, and the difference was statistically significant(P<0.05).The total efficacy of the test group was higher than that of the control group.ConclusionBoth traditional Chinese medicine fumigation and washing combined with intradermal needling and simple intradermal needling can effectively improve the clinical symptoms of patients with wind-cold and dampness type of scapulohumeral periarthritis. However, traditional Chinese medicine fumigation and washing combined with intradermal needling is more effective in alleviating the patients’ pain, improving their shoulder joint function and their wind-cold symptoms, which is simple to operate, with minimal side effects and significant therapeutic effects. |
