The Research Of Clinical Experience And Medication Rule Of Professor Zhang Chao In Treating Metabolic Associated Fatty Liver Disease

Objective:The clinical case data of Professor Zhang Chao’s treatment of metabolic associated fatty liver disease were systematically collected and sorted out.The traditional Chinese medicine inheritance assistance platform system(V3.0)was adopted to excavate and sort out the prescription drugs used by the tutor for the treatment of metabolic associated fatty liver disease,analyze the characteristics of their therapeutic drugs,and summarize the rules of their compatible drugs,so as to further guide clinical practice and promote application.Promote the academic inheritance of TCM.Methods:Data of 218 patients with metabolic associated fatty liver disease treated by Professor Zhang Chao from May 2020 to January 2023 were collected and sorted out,medical records that met research standards were included for standardized sorting,and the medical records were input into the data template of Traditional Chinese Medicine Inheritance Auxiliary Platform system(V3.0)to establish a database.Through statistical analysis module,TCM syndrome type statistics,four qi and five tastes of drugs,meridian conversion and efficacy statistics were carried out.Symptom,tongue and pulse,drug frequency statistics,association rules and cluster analysis statistics were carried out using the symptom,tongue and pulse analysis module,drug frequency statistics,and the core group of prescriptions were mined for network view display.According to the results,the compatible medication rule of Professor Zhang Chao in the treatment of metabolic associated fatty liver disease was analyzed and summarized,and the clinical experience of Professor Zhang Chao in the treatment of this disease was further summarized.Result:1.Among the 218 patients collected in this study,156 were male(71.56%)and 62 were female(28.44%).The disease occurs in people aged 30-39 years,accounting for33.94%.Followed by 40-49 years old and 20-29 years old,accounting for 24.31% and22.01%,respectively.Metabolic associated fatty liver disease is often associated with other diseases,of which 10 diseases are more than or equal to 2 times,and the top 5 diseases with the frequency are hyperuricemia,hyperlipidemia,obesity,hypertension and diabetes,respectively.2.The study found that among 395 medical records,the top 10 symptoms of patients with metabolic associated fatty liver disease were fatigue,insomnia,hypocholic pain,loose stool,bitter mouth,dry mouth,irritable temperament,dizziness,dizziness,loose stool.Tongue quality to light tongue,tooth marks tongue,tongue coating is more in white greasy moss.The pulse is mainly smooth,smooth and smooth.According to the statistical order of frequency of TCM syndromes,the following are syndrome of stagnation of dampness and turbidity,syndrome of liver depression and spleen deficiency,syndrome of phlegm and blood stasis interjunction,syndrome of dampness-heat accumulation and deficiency of spleen and kidney.3.The drugs with high frequency used by Professor Zhang Chao in the treatment of metabolic associated fatty liver disease include Radix japonica,poria cocos,kudzu root,stir-fried atractylodes,astragalus,Shenqu,turmeric,curcuma zedoaria,red peony root,cuscuta seed,hawthorn,Xianhecao,sand kernel,Zhixiangfu,etc.The frequency of drug use in accordance with the five tastes from high to low were sweet,sweet,bitter,sour and salty.The four qi categories from high to low are warm,flat,cold,cool and hot.And for the return to the spleen,stomach,liver meridian mainly.The efficacy classification of the drugs used were highest in tonifying deficiency and promoting blood circulation and removing blood stasis.The analysis of drug association rules showed that the most closely related drug combinations were pueraria root-cuscuta seed-Rhizoma fortunei,and the combination of Radix atractylodes,turmeric,Godqu,Astragalus,poria cocos,Rhizoma curcuma and Rhizoma fortunei.Core drugs 10 flavors,for Astragalus,North Sea ginseng,fried white art,poria cocos,red peony root,pueraria,magic Qu,turmeric,Curcuma curcuma,cuscuta.Five groups of prescriptions were obtained by cluster analysis,which were:(1)Poria cocos-Curcuma curcuma-Psammosa Rhizome-Shenqu-turmeric-Stir-fried white Rhizoma;(2)Poria cocos-pueraria root-Red Peony root-fried white Rhizoma atractylodes-Divine Qu-North Sea Ginseng;(3)Cuscuta seed-fried white art-Poria cocos-turmeric-Chinese ginseng-pueraria root;(4)North Sea Ginseng-Poria cocos-Pueraria root-Astragalus membranaceus-fried atractylodes sinensis-turmeric;(5)The core drugs were mainly rhizome,rhizome,turmeric,zedoary and red peony root.Core drugs were excluded from 395 prescriptions,and other drugs were uploaded for cluster analysis again.The cluster number was set to 5,and 5 drug compatibility combinations were obtained,which were:(1)Stir-fried bupleurum bupleurum,oyster,hawthorn,Zhixiangpu,Xianhecao and blue ginseng.(2)Tangerine peel-fried scutellaria scutellaria-Pinellia pinellia-fried Coptis chinensis-Hawthorn-Crane grass;(3)Hawthorn-lotus leaf-chrysanthemum-Broxibustion herb-oyster-Fairy crane grass;(4)Hawthorn-Fairy crane grass-broxibustion-oyster-spider-little Red ginseng;(5)Hawthorn-Poria cocos-Rhizome-Pinellia officinalis-Gentiana.Conclusion:Through consultation with doctors and summary analysis of medical cases,Professor Zhang Chao’s treatment of metabolic associated fatty liver disease has formed a recognition disease,combining disease and syndrome;Syndrome differentiation of diseases,with "three elements" as the main line;Treatment of diseases,the spleen and stomach as the core;In terms of drug use,the main academic thoughts and clinical experience are guided by the three combinations.The results of data mining showed that the prescription of Professor Zhang Chao in the treatment of metabolic associated fatty liver disease had a certain regularity,and it was unique in four qi,five taste,normalization and commonly used drug combination.The drugs with high frequency of use are Radix salva,poria,Pueraria,stir-fried white art,Astragalus,Shenqu,turmeric,Curcuma,Radix paeoniae,cuscuta,Hawthorn,crane grass,amomum kernel,stir-fried incense and so on.From high to low,the four qi are warm,flat,cold,cool and hot.From high to low,the five tastes are sweet,sweet,bitter,sour and salty.As for the normalization of the channel,it is mainly to the spleen,stomach and liver channels.The efficacy classification of the drugs used was the most frequent for tonifying deficiency and promoting blood circulation and removing blood stasis.The characteristics of its therapeutic drugs are:(1)treatment based on syndrome differentiation and flexible use of drugs;(2)send prescription medicine,pay attention to care for the spleen and stomach;(3)The combination of tradition and modernity improves the accuracy of medication;(4)Traditional Chinese medicine,pay attention to the use of ethnic medicine.It provides reference experience for the clinical treatment of this disease.
