To Explore The Traditional Chinese Medicine Syndrome Types Of Perimenopausal Depression Based On Infrared Thermal Imaging Technology

Objective:Using infrared thermography,the preliminary investigation of the changes in infrared thermogram and infrared temperature of different TCM evidence types in patients with perimenopausal depression provides a visual basis for TCM diagnosis of perimenopausal depression.Method:The cases were collected from the outpatient clinic of the Department of TCM,the Department of Psychology and the health check-up centre of Kunming TCM Hospital from December 2021 to December 2022 from women aged 40-60 years old.In this study,100 cases were collected from the observation group(women who met the diagnosis of perimenopausal depression)and 100 cases from the control group(women in perimenopause at the same period of health check-up);and the case samples in the observation group were divided into 4 groups according to the TCM classification criteria.The infrared thermograms of all subjects that had been collected were observed and summarized,and then the mean infrared temperature values of the observation sites were derived.All data collected were entered into an Excel sheet and statistical analysis was performed using SPSS 26.0 statistical software.Result:1.Infrared thermal imaging between the two groups:The infrared thermograms of the observation group were characterized by abnormal thermal metabolic maps,which were mainly manifested in(1)the double kidney region showing thermal deviation(liver-kidney yin deficiency group)or cool deviation(spleen-kidney yang deficiency group,heart-spleen two deficiency group);(2)the double hypochondrium region showing thermal deviation(liver-kidney yin deficiency group,liver qi stagnation group)or cool deviation(spleen-kidney yang deficiency group,heart-spleen two deficiency group);(3)the Ren vein showing thermal deviation(liver-kidney yin deficiency group,liver qi stagnation group)or cool deviation(Spleen-kidney-Yang deficiency group,Heart-Spleen two deficiency group);(4)the Directing Vessel presents heat deviation(Liver-Kidney Yin deficiency group,Liver-Qi stagnation group)or cool deviation(Spleen-Kidney Yang deficiency group,Heart-Spleen two deficiency group);(5)the frontal region presents heat deviation(Liver-Kidney Yin deficiency group,Liver-Qi stagnation group,Heart-Spleen two deficiency group)or cool deviation(Spleen-Kidney Yang deficiency group);(6)the Void Lane presents heat deviation(Liver-Kidney Yin deficiency group,Liver-Qi stagnation group)or cool deviation(Spleen-Kidney Yang deficiency group,heart and spleen deficiency group);(7)large abdomen and small abdomen showed cool deviation(liver and kidney yin deficiency group,spleen and kidney yang deficiency group,heart and spleen deficiency group).In the observation group,the thermal structure was disordered,with uneven distribution of heat sources and the local presence of obvious point-piece or mass-like high-temperature areas or low-temperature areas.The control group was characterized by a well-defined heat source hierarchy,basic symmetry between the left and right of the heat source,no obvious local point-like or mass-like high-temperature area or low-temperature area,and no obvious cold-heat mismatch structure.2.Comparison of average infrared temperature between the two groups:The frontal part of the observation group was higher than that of the control group(P < 0.05),and the abdomen and abdomen of the observation group were lower than that of the control group(P < 0.05).There was no significant difference between Ren vein,Du vein,Xu Li,double flank and double kidney area(P > 0.05).3.Comparison between the temperature values of double kidney area,double hypochondriac area,great abdomen,lower abdomen,Ren vein,Du vein,forehead and Xu Li among different syndrome types and the control group:In liver and kidney Yin deficiency group,the temperature in double kidney area,double flank area,Ren vein,Du vein,forehead and Xu Li were higher than those in control group(P < 0.05).There was no significant difference in abdominal temperature(P > 0.05).In the liver qi stagnation group,the temperature in the double hypocholic region,Ren pulse,Du pulse,forehead and Xu Li were higher than those in the control group(P < 0.05).The abdominal and abdominal temperatures were lower than those of control group(P < 0.05).There was no significant difference between the two kidney regions(P > 0.05).Spleen-kidney Yang deficiency group: the temperature of double kidney area,double flank area,big abdomen,lower abdomen,Ren pulse,Du pulse,forehead and Xu Li were lower than that of control group(P < 0.05).In the Xinpi and spleen deficiency groups,the temperature in the double kidney area,double flank area,big abdomen,small abdomen,Ren pulse,Du pulse and Xu Li were lower than those in the control group(P < 0.05).The frontal part was higher than that of control group(P < 0.05).4.Comparison of infrared temperature in different regions between different TCM syndrome types:The temperature of the governor vein,double kidney area and double flank area in the liver and kidney Yin deficiency group was higher than that in the other three groups(P < 0.05).The frontal temperature of liver qi stagnation group was higher than that of the other three groups(P < 0.05).In spleen-kidney Yang deficiency group,the temperature of the governor pulse,the double kidney area,the abdomen,the abdomen and the forehead were lower than those in the other three groups(P < 0.05).The deficiency content of Xinpi and spleen deficiency groups was lower than that of the other three groups(P < 0.05).Conclusion:1.In the liver and kidney yin deficiency group,the main manifestations of infrared thermography are deviations in the bilateral kidney region,the bilateral hypochondrium region,the Directing Vessel,the Ren Vessel,the frontal region,and the deficient internal heat;in the liver qi stagnation group,the main manifestations of infrared thermography are deviations in the bilateral hypochondrium region,the frontal region,the Ren Vessel,the Directing Vessel,the deficient internal heat,and cool deviations in the large and small abdomen;The infrared thermogram of the spleen and kidney Yang deficiency group is dominated by cool deviations in the bilateral kidney region,the Directing Vessel,the greater abdomen,the lesser abdomen,the bilateral hypochondrium,the Ren Vessel,the frontal region,and the deficiency of the heart and spleen.2.Patients with perimenopausal depressive status with different TCM syndrome types have characteristic manifestations in infrared thermograms and infrared temperature distribution at different sites,indicating that infrared thermography technology has guiding significance for TCM syndrome differentiation of perimenopausal depressive status and can provide visual and objective basis for syndrome differentiation and classification of perimenopausal depressive status.
