Objective: To compare the short-term clinical effects of mobile-bearing and fixedbearing prostheses in unicompartmental knee arthroplasty for medial knee osteoarthritis.Methods: A total of 50 patients who underwent unilateral unicompartmental knee arthroplasty in the Department of Orthopaedics,Yijishan Hospital of Wannan Medical College from December 2020 to November 2021 were selected as the research objects.According to the type of unicompartmental knee prosthesis used,they were divided into mobile platform(MB)group and fixed platform(FB)group,with 25 cases in each group.The general clinical data of the patients including gender,surgical side,age,and body mass index were collected.The perioperative data including operation time,intraoperative blood loss,and length of hospital stay were compared between the two groups.The hip-knee-ankle(HKA)Angle was recorded before and after operation,and the change of HKA Angle was calculated to compare the correction of lower limb alignment between the two groups.The VAS score,HSS score and knee range of motion(ROM)were compared before operation,6 months after operation and 12 months after operation to evaluate the recovery of knee joint function in the two groups.The forgotten joint score(FJS)was compared 6 months after operation and 12 months after operation to evaluate the subjective feeling of the two groups.Complications were recorded during follow-up.Results: There was no significant difference in general clinical data,perioperative data,preoperative HKA Angle,VAS score,HSS score,and knee ROM between the two groups(P>0.05).After operation,the HKA Angle and the correction degree of lower limb alignment in MB group were greater than those in FB group,and the differences were significant(P<0.05).There were significant differences in VAS score,HSS score,and knee ROM between the two groups at 6 months and 12 months after operation(P<0.05),but there were no significant differences in VAS score,HSS score,and knee ROM between the two groups at 6 months and 12 months after operation(P>0.05).The FJS score of MB group was slightly higher than that of FB group at 6 months after operation,but the difference was not statistically significant(P>0.05).The FJS score of MB group was significantly higher than that of FB group at 12 months after operation,and the difference was statistically significant(t=10.074,P<0.001).There were significant differences in VAS score,HSS score,knee ROM,and FJS score between 12 months after operation and 6 months after operation(P<0.05).There was one case of delayed wound healing in MB group,and the other cases had no complications.Conclusion: Both mobile-bearing unicompartmental knee prosthesis and fixed-bearing unicompartmental knee prosthesis can effectively improve the knee function of patients with medial knee osteoarthritis.The mobile-platform unicompartmental prosthesis has more advantages in correcting the lower limb alignment.The mobile-bearing unicompartmental prosthesis has a better degree of artificial joint amnesia than the fixed-bearing prosthesis in the early postoperative period,which can make the patients obtain better early subjective feeling... |
