Objective: 1.Meta of nourishing yin,clearing heat and activating blood circulation in Treatment of T2DM: By reading high-quality RCT literature on the treatment of T2 DM by nourishing yin,clearing heat and activating blood,it provides evidence-based medicine for clinical practice,which is conducive to the inheritance and development of empiric treatment methods of famous old Chinese medicine practitioners.2.Mechanism of Xiaoke‘an Decoction: Using network pharmacology method,multidimensional analysis explores the pharmacological mechanism of Professor Nan Zheng’s decoction of Xiaoke’an Decoction in the treatment of type 2 diabetes(T2DM).Methods: 1.Meta of nourishing yin,clearing heat and activating blood circulation in Treatment of T2DM: We need to search for literature on the treatment of type 2 diabetes mellitus with nourishing yin,clearing heat and activating blood circulation in CNKI database,Wanfang database,VIP database,Pub Med,and Cochrane,.March 2023.The experimental data required for inclusion were extracted,and the literature was meta-analyzed using STATA14 software.2.Mechanism of Xiaoke‘an Decoction: The active ingredient compound database of Xiaoke’an Decoction was established by searching TCMSP and HERB Materia Medica Group Database.Apply the Swiss Target Prediction to obtain the target of the compound.Search the Gene Cards,OMIM,and Drug Bank databases to obtain T2DM-related targets.Obtain the intersection targets of Xiaoke‘an Decoction and type 2 diabetes through the Venny2.1 website,and build a PPI network.Metascape database was used for GO analysis and KEGG pathway enrichment analysis.Result: 1.Meta of nourishing yin,clearing heat and activating blood circulation in Treatment of T2DM: literature screening: 2378 literatures were retrieved,and 13 literatures were finally included.A total of 1476 patients with type 2 diabetes mellitus included 744 cases in the treatment group and 732 cases in the control group.Meta results: clinical effective rate OR=3.536,95%CI[2.532,4.939],Z=7.41,P=0.000<0.05;fasting blood glucose WMD =-1.074,95%CI[-1.500,-0.648],and z =4.94,P=0.000<0.05;postprandial blood glucose WMD=-1.705,95%CI[-2.391,-1.018],and z=4.87,P=0.000<0.05;glycosylated hemoglobin WMD=-1.143,95CI[-1.875,-0.229],and z=2.98,P=0.003<0.05;the above results show that the method of nourishing yin,clearing heat and activating blood has a significant effect on the treatment of type 2 diabetes.2.Mechanism of Xiaoke‘an Decoction: 237 active ingredients of Xiaoke’an Decoction and485 intersection targets of Xiaoke’an Decoction and type 2 diabetes were obtained.The main targets are: ALB,AKT1,TNF,GAPDH,TP53,etc.The key active ingredients of Xiaoke’an Decoction for the treatment of type 2 diabetes are: B2-kaempferol,B1-quercetin,C1-betasitosterol,C2-Stigmasterol,etc.;the main action pathways are: PI3K-Akt signaling pathway,Ras signaling pathway,MAPK signaling pathway,Rap1 signaling pathway,c AMP signaling pathway,etc.Conclusion: 1.Meta of nourishing yin,clearing heat and activating blood circulation in Treatment of T2DM: By comparing the clinical effective rate,fasting blood glucose,postprandial blood glucose,glycosylated hemoglobin and other outcome indicators,the effect of nourishing yin,clearing heat and activating blood circulation is better than that of metformin control group.2.Mechanism of Xiaoke‘an Decoction: Through the prediction of network pharmacology,it was found that Xiaoke’an Decoction has the effects of improving insulin resistance,antioxidative stress,anti-inflammation,and hypoglycemia.
