Objective: The purpose of this study is to preliminarily clarify the clinical efficacy and safety of Jie jie chu bi TCM comprehensive program in the treatment of KOA through a randomized controlled clinical study under the guidance of Jie jie chu bi treatment theory,in order to provide research proof for the promotion and application of this program. Methods: A total of 72 patients with knee osteoarthritis who were treated in the Department of Massage,Affiliated Hospital of Changchun University of Traditional Chinese Medicine from December 2021 to December 2022 were recruited in this study. The 72 patients were randomly divided into the treatment group and the control group, with 36 cases in each group. The treatment group was given the comprehensive TCM program of relieving rheumatism, and the control group was given the exercise therapy and fascial knife treatment. The above treatments were given once every other day,20 minutes each time,3times as a course, and a total of 2 courses of treatment were given.Respectively before the start of the trial, after the first treatment, and after the test, record WOMAC score, two groups of patients JOA score and overall curative effect evaluation, using the gait analyzer test before and after therapy plantar impulse percentage and record.Comparing different time node WOMAC score,JOA score, plantar impulse percentage changes,to observe the KOA patients level of pain, joint stiffness, joint function, the improvement of the plantar pressure distribution situation, from the aspects of clinical symptoms and biomechanical characteristics of the bi removal solution to evaluate the clinical curative effect of TCM comprehensive solution. Results: Two groups of patients after treatment of WOMAC score,JOA score was significantly improve the;Two groups of patients with forefoot impulse decreases, heel impulse increases, and the overall efficacy of treatment group is better than the control group.(1)the overall clinical curative effect comparison: treatment group total effective rate(94.11%)than the control group total effective rate(88.23%),and the difference is statistically significant(P < 0.05).(2)Comparison of WOMAC scores: The pain score, stiffness score, functional score and total score of the two groups were significantly decreased after the first treatment and after the end of treatment, with statistical significance(P<0.05).Through statistical analysis, there were no significant differences in pain score, stiffness score, functional score and total score before treatment(P>0.05),and they were comparable. The difference of pain score, function score and total score after the first treatment was significant and statistically significant(P<0.05),but the difference of stiffness score was not statistically significant(P>0.05).Two groups of patients after the treatment, pain score, stiffness score, the difference of function score and total score significantly, and were statistically significant(P< 0.05),and the rigid grading control group is better than that of the treatment group.Pain scores, functional scores and total scores in the treatment group were better than those in the control group.(3)Comparison of JOA scores: Compared with before treatment,JOA scores of patients in both groups increased significantly after the first treatment and after the treatment.Through statistical analysis, two groups of patients in three time node JOA score, there is a statistically significant difference(P < 0.05),and the same time the performance of the treatment group are better than the control group(P < 0.05).(4)Comparison of plantar impulse percentage: after the treatment, the percentage of heel impulse increased and the percentage of forefoot impulse decreased in the two groups, and the curative effect of the treatment group was better than that of the control group, and the difference was statistically significant(P<0.05).There was no statistically significant difference in the percentage of midfoot impulse between the two groups(P>0.05).Conclusion: Through this study,it was found that the comprehensive program of Jie jie chu bi traditional Chinese medicine for the treatment of knee osteoarthritis has a significant effect, and has more advantages than the control group in reducing pain, improving knee joint function and comprehensive score, which provides primary clinical evidence-based basis for its effectiveness in the treatment of KOA,and further defines the advantages of the comprehensive program of Jie jie chu bi traditional Chinese medicine. It promotes the popularization and application of TCM comprehensive scheme.
