| This paper includes two parts: systematic review and clinical research.System reviewObjective: To systematically evaluate the randomized controlled literature of tuina combined with needle knife in the treatment of knee osteoarthritis in recent 20 years,and to provide evidence-based medical evidence for formulating the scheme of tuina combined with needle knife in the treatment of knee osteoarthritis.Methods:The literature on the clinical randomized controlled trial(RCT)of needle knife combined with tuina in the treatment of knee osteoarthritis in CNKI,Wanfang,VIP,CBM,Pub Med and Cochrane libraries were searched by computer,For the literatures that meet the inclusion criteria,the methodological quality evaluation was carried out by using the RCT quality evaluation tool recommended in Cochrane evaluation manual 5.1,and the general data and clinical effective rate of patients were extracted.Revman5 4software for meta-analysis.Results:The effective rate of tuina combined with needle knife in the treatment of knee osteoarthritis was significantly better than that in the control group [RR = 1.18,95% CI(1.14,1.22),z = 9.04,P < 0.0001];The decrease of VAS pain score was better than that of the control group,and the difference was statistically significant [MD =-1.16,95% CI(-2.48,0.17),z = 8.86,P < 0.0001];Three studies reported no obvious adverse reactions;Most of the selected parts of the needle knife are anatomical parts,and 12 kinds of massage techniques are used,mainly including tendon manipulation and restoration techniques such as pulling and stretching,flexion and extension.Conclusion: Tuina combined with needle knife is a safe and effective method in the treatment of knee osteoarthritis.Clinical researchObjective: To observe the clinical efficacy of tuina combined with needle knife in the treatment of knee osteoarthritis by using VAS score,WOMAC score and infrared thermal imaging technology,in order to provide a safer and effective non-surgical treatment scheme for knee osteoarthritis.Methods: 60 volunteers diagnosed with knee osteoarthritis were randomly divided into tuina combined with needle knife treatment group and simple needle knife control group,with 30 cases in each group.Before and after treatment,VAS score,WOMAC score and infrared thermal imaging technology were used to evaluate the clinical efficacy of the two groups,and SPSS26.0 software for analysis.Results: Both groups had definite curative effects on koa,but the curative effect of the treatment group was better;After treatment,VAS score,WOMAC score and infrared thermal imaging temperature in the two groups decreased compared with those before treatment(P < 0.05);After treatment,the VAS score,WOMAC score and infrared thermal imaging temperature of the treatment group decreased significantly compared with the control group(P < 0.05).The total effective rate of the treatment group was 96.67% higher than 83.33% of the control group.Conclusion:Compared with simple needle knife treatment,tuina combined with needle knife treatment can significantly reduce patients’ knee pain,improve patients’ knee joint functional activities,accelerate local blood circulation and promote the regression of inflammation,with less side effects and better clinical efficacy. |
