Clinical Study On The Intervention Of Huotan Jiedu Huaban Decoction On Stable Anginapectoris Complicated With Atheroscleroticplaques Of Lower Limbs (Phlegm-Stasis Interjunction Type)

Objective:In this study,the clinical efficacy of Huotan Jiedu huaban Decoction onpatients with stable anginapectoris complicated with lower limbs atherosclerosis(phlegm-stasis interjunction type)was observed to objectively evaluate its safety and effectiveness.To provide a new method and strategy for the prevention and treatment of coronary heart disease and lower limb atherosclerosis with traditional Chinese medicine.Methods: A total of 72 patients who met the inclusion criteria were selected from the heart disease Department of the Affiliated Hospital of Changchun University of Chinese Medicine(Jilin Hospital of Traditional Chinese Medicine).They were randomly divided into the treatment group and the control group,with 36 patients in each group.The basic data of the two groups before treatment were comparable.The control group was given conventional Western medicine treatment(β-blockers,nitrates,antiplatelet drugs,lipid-lowering drugs,if necessary to improve limb ischemia and other drugs);On the basis of the control group,the treatment group was given a self-designed Chinese herbal prescription Huotan Jiedu Huaban tang(specific drugs are: Honeysuckle 30 g,angelica 15 g,Xuan Shen 15 g,raw licorice 10 g,leeches 5g,Coptidis 10 g,Zhi Mu,10 g,Salvia miltiorrhiza 15 g,Ligusticum Chuanxiong 15 g,Trichosanthes 10 g,Pinellia pinellia 10 g,frankincense 10 g,myrrh 10g),twice a day,orally in the morning and evening.The course of treatment was 24 weeks.Angina pectoris symptoms of the treatment group and the control group were observed and compared,the femoral artery intima-media thickness(IMT),maximum plaque area,Crouse score,ankle-brachial index(ABI),TCM symptom score,lipid indexes(TG,TC,LDL-C,HDL-C),lipoprotein α(Lp(α))detected by lower limb artery color Doppler ultrasound.Changes in hypersensitive C-reactive protein(hs-CRP),serum homocysteine(Hcy),and safety markers.Data before and after treatment were collected,and statistical analysis was performed to evaluate the efficacy.Results: There were no significant differences between the two groups before treatment in general condition,angina pectoris score,TCM syndrome score,lower extremity arterial plaque IMT,maximum plaque area,Crouse score,ABI,blood lipid,Lp(α),Hcy and hs-CRP(P > 0.05),indicating comparability.After 24 weeks of treatment,angina pectoris and TCM symptoms were relieved in both groups by intra-group comparison,IMT,maximum plaque area and Crouse score of lower extremity artery were significantly improved,ABI was decreased,TG,LDL-C,Lp(α),Hcy and hs-CRP were decreased,and the differences were statistically significant(P < 0.05).After comparison between the two groups,the treatment group was better than the control group,the difference was statistically significant(P < 0.05).Conclusion: 1.Huotan Jiedu huaban Decoction is effective and safe for patients with stable angina pectoris complicated with lower limb atherosclerosis(phlegm-stasis interjunction type);2.Huotan Jiedu Huaban Decoction combined with Western medicine treatment can significantly improve the symptoms of angina pectoris and the clinical syndrome of traditional Chinese medicine,as well as reverse the lower extremity artery plaque,reduce blood lipids,anti-inflammatory and anti-arteriosclerosis,and its effect is obviously superior to that of Western medicine treatment alone.
