OBJECTIVE:To observe the effect of intravenous sedation with ciprofol and dexmedetomidine on perioperative stress response in patients undergoing ankle osteotomy surgery.METHODS:A prospective study method was used to include 40 patients(21 males and 19females)who underwent ankle osteotomy surgery under combined spinal-epidural anesthesia in the afternoon of July to September 2022 in our hospital.They were randomly divided into ciprofol group(group C)and dexmedetomidine group(group D)with 20 cases in each group.Group C was given 0.25 mg/kg of ciprofol(injection time 30 s),and then continuously pumped at 0.6 mg·kg-1·h-1.Group D was given a loading dose of dexmedetomidine 0.8μg/kg(micropump infusion for 10 min),and then continuously pumped at 0.2μg·kg-1·h-1.The Narcotrend index(NI),mean arterial pressure(MAP)and heart rate(HR)of the two groups were recorded before treatment(T0),10 min after treatment(T1),20 min after treatment(T2),30 min after treatment(T3)and at the end of operation(T4).Venous blood was taken at T0,T4and the day after operation to determine plasma noradrenaline(NE)and serum cortisol(Cor)levels,and the degree of forgetting and the incidence of adverse reactions were recorded.RESULTS:The MAP of the ciprofol group was significantly lower than that of the dexmedetomidine group(P<0.05);NI in the ciprofol group was significantly higher than that in the dexmedetomidine group(P<0.05);At T4,the contents of NE and Cor in the ciprofol group were significantly lower than those in the dexmedetomidine group(P<0.05);The incidence of bradycardia in the ciprofol group was significantly lower than that in the dexmedetomidine group(P<0.05);The incidence of respiratory depression in the ciprofol group was significantly higher than that in the dexmedetomidine group(P<0.05).CONCLUSION:As an auxiliary sedative for combined spinal-epidural anesthesia,ciprofol has an effective and safe sedative effect in clinical application,which can effectively reduce the perioperative stress response of patients undergoing ankle osteotomy surgery.Compared with dexmedetomidine,ciprofol has a more outstanding effect in reducing stress response,providing new ideas and methods for the comfort of clinical anesthesia and rapid postoperative rehabilitation of patients.
