A Study On The Funds Guarantee Of The "Medical-Maintenance Combination" In Nanjing

With the acceleration of the aging society and the miniaturization of family structure,the demand of the elderly for the service of "combination of medical care and nursing care" is increasing.But at the moment,"combination of medical and nursing care" endowment institutions is difficult to fit into the elderly expectations,existing geriatric nursing home nature,scope of service management,access mechanism,evaluation standard specification is not clear,pension service category not diversity,capital problem such as security system is not sound,among them,the fund security system built entirely by "combination of medical and nursing care" pension system construction,the basis of solving good "combination of medical and nursing care" funding problems,is conducive to the sustainable development of the model.This paper uses methods of literature analysis,case analysis and comparative analysis,combined with the market failure theory,public goods theory and the theory of welfare pluralism in Nanjing "combination of medical and nursing care" endowment institutions as the key object of study,in-depth discussion and research on their funding problems.This paper mainly analyzes the development status,existing problems and relevant experience at home and abroad of the "combination of medical care and old-age care" institutions in Nanjing.First of all,this paper analyzes the development status of the "combination of medical care and nursing care" pension service institutions in Nanjing from four aspects:institution setting,operating subject,scale classification and reaching model.Secondly,through the analysis of the exploration experience in the aspect of financial security,it is concluded that there are the following problems in Nanjing’s institutional financial security of "combination of medical care and nursing care" : the long-term care insurance system is not perfect;The financing channels are single and the development of commercial long-term care insurance is slow.The waste of medical funds is serious and the operation efficiency is low.Finally,based on the domestic and foreign experience of Japan,the United States,Germany,Qingdao,Chengdu and other places,combined with the current situation and existing problems of Nanjing’s "combination of medical care and old-age care" model,this paper puts forward countermeasures and Suggestions from four aspects,namely,improving the system,increasing channels,improving management,and encouraging social participation.The research conclusion of this paper is as follows: the key to solve the problem of "combination of medical care and nursing care" in Nanjing city lies in the two aspects of "increasing revenue" and "reducing expenditure".In terms of "open source",first of all,the long-term care social insurance system should be established and improved,such as formulating and implementing scientific social insurance system,introducing perfect laws and regulations,effectively standardizing the financing behavior of "combination of medical care and nursing care",and transferring the basic medical insurance as an important source of the long-term care insurance fund of Nanjing City.Secondly,we should increase the subsidy intensity and funding channels for the combination of medical care and medical care,including providing special funds for the combination of medical care and medical care,establishing and improving the government subsidy system,improving the accuracy of the subsidy system,deeply tapping the potential of the social security fund,and expanding the channels of donation and investment.Third,encourage and guide social forces to participate in the "combination of medical care and old-age care" model,such as the government to attract social forces to join the establishment of institutions,to provide quality services for the public or the government to provide financial support for the "combination of medical care and old-age care" institutions that have not been established or have been established.In terms of "reducing expenditure",relevant departments of Nanjing should establish a more strict management review system,strengthen the use and management of "combination of medical care and nursing care" funds,resist the possible inflation problem to ensure the maintenance and appreciation of funds,and effectively ensure the safe operation of social security funds.
