| Objective: This study was conducted to observe the clinical efficacy of the method of dispelling wind, resolving phlegm and promoting ligament combined with auricular acupressure of beans in the treatment of phlegm and stasis blocking migraine,to observe whether this method can improve the symptoms of migraine, and to study whether this method can effectively improve the clinical efficacy of migraine, with the aim of providing a safer and more effective idea and method for the clinical treatment of phlegm and stasis blocking migraine, and to enrich the Chinese medical treatment options for headache. Methods: Patients who were qualified for this study were gathered from September 2021 to December 2022 on the basis of their informed consent. The Affiliated Hospital of Jiangxi University of Chinese Medicine provided a total of 60 patients with phlegm-stasis-blocking migraine who satisfied the criteria for this study. These patients were divided into two groups by lottery, with 30 instances being treated and 30 cases being left untreated. In the treatment group,a 200 ml dose of Chinese herbal medicine was taken twice a day,in the morning and the evening,to clear the ligaments, clear the wind, and resolve phlegm. Moreover, auricular acupuncture point pressure was applied externally.Flunarizine hydrochloride was given orally to the control group as 1(5 mg)capsule each night, which could be raised to 10(10 mg)capsules as needed.Four weeks of treatment were given to both groups. The headache score,VAS score and total TCM symptom score were quantified before and after the intervention, and adverse events were observed and recorded, and the data were statistically analysed using SPSS 26.0 software to produce the corresponding results to assess the clinical efficacy.Results(1)In terms of basic conditions: one patient was excluded from the control group because she switched to other medication en route, one patient was excluded from the observation group because she could not adhere to herbal medicine, and one patient was excluded because she failed to perform auricular acupressure regularly.A total of 57 patients were eventually recruited,of which 28 and 29 were in the observation group and the other were in the control group. For baseline information(length of sickness, age, gender, and severity of disease),there was no discernible difference between the two groups(P > 0.05),and they were equivalent.(2)In terms of VAS scores, the two groups were comparable before the intervention, with the observation group’s VAS score being 7.90±1.10 and the control group’s being 7.97±1.43(t =-0.119 by t-test,P =0.906>0.05).Following the intervention, the two groups’ VAS scores were 3.46±1.3 for the observation group and 4.69±1.58 for the control group. The VAS scores of the two groups following the intervention were 3.46±1.3 in the observation group and 4.69±1.58 in the control group, with z=-0.91 by rank sum test and P=0.004 < 0.05 indicating that the outcomes were substantially different.(3)In terms of migraine score: the migraine score before intervention was 20.07±2.98 in the observation group and 20.41±2.67 in the control group,z=-0.703 by rank sum test,P=0.482>0.05,no significant difference, the two groups were comparable;in terms of migraine score after intervention,11.07±3.09 in the observation group and 13.21±3.28 in the control group,t=-2.529 by t-test,P=0.014<0.05,the results were significantly different. In terms of the post-intervention migraine score, the observation group was 11.07±3.09 and the control group was13.21±3.28,with t =-2.529 by t-test,P = 0.014 < 0.05,which was a significant difference. In addition,in terms of the main symptoms of migraine, the two groups were comparable before the intervention(P >0.05).After two weeks of intervention, the symptoms of both groups improved(P < 0.05),and the efficacy of the observation group was better than that of the control group(P < 0.05).(4)In terms of the total TCM symptom score: the total TCM symptom score of the two groups before the intervention was 16.00±2.02 in the observation group and 16.38±1.94 in the control group,t =-0.724 by t-test,P = 0.472 > 0.05,no significant difference, the two groups are comparable. In the comparison of the two groups, the total TCM symptom score was 4.96±2.99 in the observation group and 7.83±4.37 in the control group, with z=-2.418 by rank sum test,P=0.016<0.05,the results were significantly different. In addition in terms of core TCM symptoms, the two groups were comparable before the intervention(P > 0.05),after the intervention, both groups showed improvement in symptoms(P < 0.05),and the efficacy of the observation group was better than that of the control group(P < 0.05).(5)During a 4-week treatment period, the observation group’s overall clinical efficacy(92.86%)was higher than the control group’s(72.41%),and there was a statistically significant difference between the two groups(P<0.05).The observation group outperformed the control group in terms of the effectiveness of TCM symptoms(92.86% vs.68.97%),and statistical analysis revealed a significant difference between the two groups(P<0.05).This suggests that in terms of clinical effectiveness, the observation group outperformed the control group.(6)During the treatment process of this study, the results of adverse reactions were observed as follows: one case of gastrointestinal reaction occurred in the control group during the treatment period, while no patient with adverse reactions occurred in the observation group;no significant abnormalities were found in the three major routine tests, coagulation function, electrocardiogram and other basic tests in both groups. Conclusion: The treatment method of dispelling wind, resolving phlegm and clearing ligaments combined with ear acupressing beans can effectively improve the pain level, frequency of attacks and Chinese medicine symptoms in patients with phlegm and stagnant ligaments migraine, and its efficacy is better than that of Western medicine treatment protocols alone;at the same time,in terms of safety,no adverse reactions or aggravation of the disease occurred in the observation group of this researcher, confirming that it is worthy of further in-depth study. |
