| Background:Obesity is often accompanied by abnormal metabolic markers such as high cholesterol and high triacylglycerol.Among these abnormalities,LDL-C/HDL-C can provide a more comprehensive risk assessment compared to low density lipoprotein-cholesterol(LDL-C)or high density lipoprotein-cholesterol(HDL-C)alone.provide a more comprehensive risk assessment.In addition,LDL-C/HDL-C has been shown to be superior to LDL-C and HDL-C in the assessment of cardiovascular disease risk,so LDL-C/HDL-C is of great value in the management of obese patients.Currently,laparoscopic sleeve gastrectomy(LSG)has become an effective treatment for obesity,significantly reducing body weight,improving insulin resistance and regulating lipid metabolism.However,the effect of LSG on LDL-C/HDL-C remains unclear,and the results of studies on the effect of the procedure on relevant metabolic markers are inconsistent.Therefore,studying the effect of LSG on LDL-C/HDL-C ratio and related metabolic indexes in obese patients can help evaluate the effect of the procedure on the treatment of obesity and related complications,and provide a scientific basis for the clinical treatment of obesity.Objective:To investigate the effect of laparoscopic sleeve gastrectomy(LSG)on LDL-C/HDL-C and related metabolic indexes in obese patients.Methods:A total of 70 obese patients who underwent LSG at our hospital from November 2018 to September2021 were collected,and the preoperative and postoperative 1,3,6 and 12 months clinical general data and relevant biochemical index data were compiled.The study population was divided into a high ratio group(2.77-8.13)and a low ratio group(0.89-2.71)by the median LDL-C/HDL-C.The patients in the high ratio group were aged 19-57 years,21 males and 14 females,with a mean of 40.11 ± 11.35 years,and the patients in the low ratio group were aged 20-56 years,16 males and 19 females,with a mean of 36.06 ± 11.10 years.The preoperative and postoperative clinical data of the two groups were retrospectively compared and analyzed to observe the trends of preoperative and postoperative changes in LDL-C/HDL-C and related metabolic indexes after LSG,to analyze the correlation between LDL-C/HDL-C and related metabolic indexes after surgery,and to compare the efficacy of LSG in patients with different LDL-C/HDL-C levels.Results:The weight and body mass index(BMI)of patients in both groups at 1,3,6,and 12 months after surgery were lower than those before surgery(P<0.05),showing a gradual decrease,and the extra weight loss rate(EWL)at 12 months after surgery were 75.31±10.94%,74.71±11.52%,respectively.The LDLC/HDL-C,LDL-C,Total Cholesterol(TC),and Triglyceride(TG)were lower than those before surgery in both groups at 1,3,6,and 12 months after surgery(P<0.05),showing a gradual decreasing trend,and the LDL-C/HDL-C of the patients in the high ratio group at 12 months after surgery increased from 4.12% in the preoperative period to 3.5% in the postoperative period.C in the high ratio group decreased from 4.12±1.46 preoperatively to 0.76±0.24,and LDL-C/HDL-C in the low ratio group decreased from 2.13±0.47 preoperatively to 1.06±0.31 at 12 months postoperatively,and HDL-C was higher than preoperatively in 1,3,6 and 12 months postoperatively in both groups(P<0.05),showing a gradual increase.The improvement of LDL-C,LDL-C/HDL-C and HDL-C indexes in the high ratio group was better than that in the low ratio group(P<0.05);Fasting Plasma Hlucose(FPG),Fasting Insulin(INS),Glycosylated Hemoglobin(Hb A1 c,Hb A1c)in 1,3,6 and 12 months after surgery in both groups were found to be better than those in the low ratio group(P<0.05).Hemoglobin A1c(Hb A1c)were all lower than those before surgery(P<0.05),showing a gradual decrease;acid(UA),alanine transaminase(ALT),aspartate transaminase(AST)were all lower than those in both groups at 1,3,6 and 12 months after surgery.Transaminase(AST)were lower than those before surgery(P<0.05),showing a gradual decrease,and when comparing between groups,it was found that the improvement of UA in the high ratio group was better than that in the low ratio group(P<0.05).By correlation analysis,a negative correlation was found between ΔLDL-C/HDL-C and ΔAST at 12 months after surgery in the high ratio group(P<0.05),in addition to a correlation between LDL-C/HDL-C and HDL-C,which was not found in the low ratio group.Conclusion:1.LSG can significantly reduce the weight of obese patients and improve LDL-C/HDL-C and related metabolic indexes,thus effectively preventing cardiovascular,cerebrovascular,glucose,liver and other diseases in obese patients;2.LSG showed better improvement in LDL-C/HDL-C,LDL-C,HDL-C,and UA in obese patients in the high ratio group than in the low ratio group;... |
