Objective:To explore the clinical value of insulin resistance replacement index,TYG index,TYG-BMI,TG/HDL-C in predicting type 2 diabetes mellitus combined with NAFLD.Methods:A total of 291 patients with type 2 diabetes who attended the Department of Endocrinology of Shanxi Provincial People’s Hospital from November 2020 to October2022 were selected as the study subjects.According to the criteria of the arrangement,the abdominal ultrasound results were taken as the standard,and the group with type 2diabetes mellitus(n=139)and type 2 diabetes mellitus combined with NAFLD(n=152)were divided.Chi-square tests were used for comparisons between groups of counting data,and two-independent sample t-test or Mann-Whitney U test were used for comparisons of metric data.Logistic regression was used to analyze the relationship between TYG index,TYG-BMI,TG/HDL-C and NAFLD in type 2 diabetes.The receiver operating characteristic curve(ROC curve)was plotted to evaluate the predicted value of TYG index,TYG-BMI,TG/HDL-C,and the cut-off value was obtained.The De Long method was used to compare whether there was a statistical difference between the ROCs of the above three indicators.Results:A total of 291 patients with type 2 diabetes were enrolled,including 152 cases in the type 2 diabetes group with NAFLD,with a detection rate of 52.3%.There were significant differences in age,ALT,AST,FINS,GGT,AKP,HDL-C,disease course,TYG index,TYG-BMI,TG/HDL-C between the type 2 diabetes mellitus group and the NAFLD group and the type 2 diabetes simple group(P all <0.05).Multivariate logistic regression analysis showed that after adjusting for confounding factors,the alternative indicators of insulin resistance TYG index,TYG-BMI and TG/HDL-C were independent risk factors for NAFLD,and the results showed that the area under the ROC curve(AUC)of TYG-BMI to predict NAFLD in patients with type 2 diabetes mellitus was significantly greater than that of TYG index and TG/HDL-C(0.803 versus 0.710,0.742).There was a statistically significant difference in AUC comparison(P < 0.05).The cut-off value of TYG-BMI for predicting NAFLD in patients with type 2 diabetes mellitus was 222.077,which was a better predictor of type 2 diabetes mellitus combined with NAFLD.Conclusion:The predictive value of TYG-BMI on NAFLD in patients with type 2 diabetes mellitus is better than that of TYG index,and TG/HDL-C can be used in clinical screening and assist in the diagnosis of type 2 diabetes combined with NAFLD.
