ObjectiveThrough the retrospective analysis of the clinical data of placental abruption and non-placental abruption normal control group,the harm of placental abruption to mothers and children was discussed,and the risk factors of placental abruption were clarified,so as to provide a basis for the prevention and clinical intervention of placental abruption.MethodsA total of 116 patients with placental abruption who were hospitalized and delivered in The Second Affiliated Hospital of Kunming Medical University from January 2018 to December 2022 were collected as placental abruption group,and 116 patients who did not experience placental abruption during the same hospital delivery were collected as the control group.The basic information of patients in the placental abruption group and control group,the clinical manifestations,signs,grade of placental abruption,other comorbidities and complications,blood routine,coagulation function test,intrapartum and postpartum conditions and neonatal prognosis were compared and analyzed by retrospective research methods,so as to clarify the harm of placental abruption to mothers and infants,and explore the risk factors for placental abruption.Results1.The placental abruption group had significantly higher rates in twin pregnancy rate,assisted reproductive technology conception,premature rupture of membranes,gestational hypertension,anemia and preterm birth than the control group,and the differences were statistically significant(P<0.05).2.The Activated Partial Thromboplastin Time(APTT)and D-Dimer(D-D)indexes in the placental abruption group were significantly higher than those in the control group.The content of fibrinogen(FIB)was significantly lower than that of the control group,and the difference was statistically significant(P<0.05).3.The gestational age and neonatal weight of the placental abruption group were significantly lower than those in the control group,and the cesarean section rate,maternal hemorrhagic shock incidence and blood transfusion rate were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).4.The mortality rates of low birth weight,preterm birth,fetal distress,neonatal asphyxia,neonatal intensive care unit(NICU),and neonatal mortality in the placental abruption group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).5.The results of multivariate binary logistic regression analysis showed that the independent risk factors for placental abruption were twin pregnancy,assisted reproduction,premature rupture of membranes,hypertensive disorders during pregnancy and anemia.According to the ratio ratio(OR value,the order is twin pregnancy,anemia,hypertensive diseases during pregnancy,assisted reproduction and premature rupture of membranes.Conclusion1.Placental abruption seriously endangers the health of the mother and fetus,and increases the risk of cesarean section,hemorrhagic shock,and blood transfusion.At the same time,low birth weight,preterm birth,fetal distress,neonatal asphyxia,neonatal conversion to NICU and neonatal mortality were significantly increased.2.Twin pregnancy,assisted reproduction,premature rupture of membranes,hypertensive disorders during pregnancy,and anemia are independent risk factors for placental abruption.
