Objective The efficacy and safety of Anti-itch lotion in the treatment of perianal eczema of syndrome of dampness-heat diffusing downward and its effect on the expression level of IL-6 in peripheral blood were observed, and the mechanism of action of Anti-itch lotion was preliminarily discussed. Methods A total of 32 patients with perianal eczema(syndrome of dampness-heat diffusing downward)from the outpatient clinic of the proctology department of Yinchuan Traditional Chinese Medicine Hospital will meet the requirements. The treatment group and the control group were grouped, and 32 cases in the treatment group and the control group were respectively smoked with Anti-itch lotion and oral gentian diarrhea liver mixture.Triamcinolone acetonide econazole cream was rubbed on the affected area and treated with oral gentian diarrhea liver mixture, and the total course of treatment was 14 days. The degree of itching, rash area, perianal exudate, rash morphology, peripheral blood IL-6 expression level and overall efficacy of patients before treatment,7 days and 14 days after treatment were observed.Follow-up for 90 days was followed for 90 days to see if there was any recurrence. The results of each group were analyzed and processed using SPSS 26.0software.Results A total of 64 cases were collected clinically,of which 4 cases were shed and eliminated, and finally 60 cases were included, and 30 cases were in the treatment group and 30 cases in the control group.1.There were no statistical differences in the basic information(sex, age),symptom score and peripheral blood IL-6 expression level of the two groups of patients before treatment(p>0.05),which was comparable.2.Observation indexes after 7 days of treatment:comparison before and after treatment within the group:the degree of itching, perianal exudate, rash morphological score and total symptom score were compared, and the differences were statistically significant(p<0.05);There was no significant difference in the integral comparison of rash area(p>0.05).After treatment, there were no significant differences in the degree of pruritus, lesion area, perianal exudate, rash morphological score and total score(p>0.05).3.Observation indexes after 14 days of treatment:comparison before and after treatment:degree of itching, skin lesion area, perianal exudate, rash morphological score, total symptom score and peripheral blood IL-6 expression water comparison, the difference was statistically significant(p<0.05).After treatment, the differences between groups were statistically significant in the degree of pruritus, perianal effusion, rash morphological score, total symptom score and peripheral blood IL-6expression level(p<0.05).There was no significant difference in the integral comparison of lesion area(p>0.05).4.Clinical efficacy:treatment group(Anti-itch lotion+gentian diarrhea liver mixture)VS control group(triamcinolone acetonide iconazole cream+gentian diarrhea liver combination),the total effective rate:93.33%VS 73.33%,there was a statistical difference in the total effective rate between the two groups(X~2=8.108,p=0.042<0.05),indicating that the overall efficacy of the treatment group was better.5.Prognosis:90 days after the end of treatment, the cured, effective and effective patients were followed, including 28 cases in the treatment group,2 cases with recurrence,22 cases in the control group,7 cases with recurrence, recurrence rates of7.14%and 31.82%,respectively,and there was a statistical difference p=0.032(p<0.05),indicating that the recurrence rate in the treatment group was lower and the prognosis was good.6.Safety evaluation:no allergies, local redness, swelling, pain and other discomforts were found during the treatment of the two groups. Conclusion 1.Anti-itch lotion fumigation combined with oral gentian diarrhea liver mixture is superior to triamcinolone acetonide iconazole cream combined with oral gentian diarrhea liver combination in the overall efficacy of the treatment of moist heat bet-type perianal eczema, which can significantly improve perianal itching, exudation, rash area and morphology, and the recurrence rate is low, which is a feasible treatment plan.2.The promotion of Anti-itch lotion provides further clinical evidence.3.Anti-itch lotion fumigation combined with oral gentian diarrhea liver mixture can significantly reduce the expression level of IL-6 in peripheral blood, and the effect is better than that of the control group.
