Objective:To investigate the effect of Taohong Siwu Decoction on the rapid recovery of elderly intertrochanteric fracture after PFNA,and to compare the clinical efficacy difference between Yunnan Baiyao Powder.Method:A total of 90 elderly intertrochanteric fracture patients treated with PFNA internal fixation in the orthopedics Department of Traditional Chinese medicine hospital Dianjiang Chongqing affiliated to Hunan University of Chinese Medicine from November 2020 to November 2021 who met the inclusion criteria were selected and randomly divided into two groups,with 45 cases in the treatment group and 45 cases in the control group.The control group was treated with basic treatment plus Yunnan Baiyao Powder,and the treatment group was treated with basic treatment plus peach blossom Siwu decoction.VAS,limb circumference difference,inflammatory indicators,Harris score,TCM syndrome score,TCM efficacy,postoperative complications and other related indicators were compared between the two groups.Results:(1)General information: there was no difference in the general information of the two groups before treatment,and the baseline was consistent and comparable;There was no significant difference in VAS score,limb circumference difference,inflammation index and TCM syndrome score between the two groups on the 1st postoperative day(P>0.05),indicating comparability.(2)On postoperative day 3 and postoperative day 7,VAS score and limb circumference difference in 2 groups were significantly improved compared with that on postoperative day 1,the differences were statistically significant(P<0.05),and VAS score and limb circumference difference in the treatment group were lower,the differences were statistically significant(P<0.05).(3)Il-6,PCT and CRP levels in 2 groups showed a decreasing trend over time on the 3rd and 7th postoperative days,with statistically significant differences(P<0.05).The levels of IL-6,PCT and CRP in the treatment group decreased more significantly on the 3rd and 7th postoperative days,and the differences were statistically significant.(4)The time of stitches removal was statistically significant between the two groups(P < 0.05),and the time of stitches removal was shorter in the treatment group.(5)Harris hip score in both groups increased significantly on the 14 th day after surgery compared with the 1st day after surgery,with statistical significance(P<0.05);The Harris score of hip joint was higher and the effect was better in the treatment group 14 days after operation,the difference was statistically significant(P<0.05).(6)TCM syndrome scores in both groups were significantly improved on the 14 th day after surgery compared with the 1st day after surgery.The total effective rate of the treatment group was 97.72%,which was significantly higher than that of the control group(88.64%),with statistical significance(P<0.05).(7)Up to 1 month after the operation,no adverse reactions such as drug allergy and liver and kidney function injury occurred in patients in both groups.Conclusion:Taohong Siwu Decoction and Yunnan Baiyao Powder used in elderly patients with qi-stagnation and blood-stasis intertrochanteric fracture after PFNA can relieve pain,reduce swelling,improve hip joint function,reduce postoperative inflammatory reaction and improve the symptoms of qi-stagnation and blood-stasis syndrome,but Taohong Siwu Decoction restore faster and has better curative effect.
