| Objective: To provide clinical evidence for the development of clinical diagnosis and treatment scheme under the guidance of "double heart treatment" mode,by observe the clinical efficacy of Jiangqi Dayu decoction combined with emotional therapy in the treatment of acute myocardial infarction of liver qi stagnation type and / or anxiety after PCI.Substances and methods: Based on the "double heart medicine" model,a randomized,blind and parallel controlled clinical trial was established.369 patients who met the inclusion criteria were included in,which visited the Department of Cardiology of the Hospital of Liaoning University of traditional Chinese medicine from June 2019 to may 2020.They were randomly divided into the experimental group,control group 1 and control group 2.The control group 2 was treated with conventional basic Western medicine,the experimental group was treated with "three-step emotional counseling" combined with "Jiangqi Dayu Decoction" on the basis of the control group 2,and the control group 1 was treated with "three-step emotional counseling" combined with placebo on the basis of the control group 2.The course of hospitalization was 10 days and the outpatient follow-up was 4 weeks.Four weeks later,the scores of TCM syndromes,Hamilton Anxiety Scale and CCS angina pectoris were followed up;The mortality,readmission rate and complication rate of each group were followed up after half a year.The results were analyzed by spss26.0 software.result: 1 TCM syndrome score:(1)Comparison of total TCM syndrome score: the experimental group and control group 1 were improved compared with that before treatment(P < 0.05),while the control group 2 had no significant change compared with that before treatment(P > 0.05);After treatment,the scores of the three groups were ranked as test group,control group 1 and control group 2 from low to high,and the difference was statistically significant(P < 0.05);(2)Comparison of total effective rate of TCM syndrome score: After treatment,the effective rate of the experimental group was 78.8%,which was higher than that of the control group 1(2.7%)and the control group 2(2.6%),the difference was statistically significant(P < 0.05),and there was no statistical difference between the control group 1 and the control group 2(P > 0.05).(3)Comparison of main symptom scores of TCM syndromes: after treatment,the main symptom scores of the three groups were lower than those before treatment(P.2 Hamilton Anxiety Scale:(1)Comparison of the total score of Hamilton Anxiety Scale: the score of Hamilton Anxiety Scale in the three groups was lower than that before treatment(P < 0.05);After treatment,the scores of the test group were lower than those of the control group 1 and the control group 2(P < 0.05),and there was no significant difference between the control group 1 and the control group 2(P > 0.05);(2)Comparison of the total effective rate of Hamilton Anxiety Scale score: after treatment,the effective rate of the experimental group was 61.9%,which was higher than that of the control group 1(1.8%)and the control group 2(3.5%).The difference was statistically significant(P < 0.05).There was no significant difference between the control group 1 and the control group 2(P > 0.05).3 Comparison of effective rate of angina pectoris grading:After treatment,the effective rate of the experimental group was 91.2%,the effective rate of the control group 1 was 40.2% and the control group 2 was 6.1%.There was significant difference between the three groups(P < 0.05).4.rehospitalization rate,complication rate and mortality:(1)Comparison of rehospitalization rate: the rehospitalization rate of the experimental group was 9.7%,which was lower than that of the control group 1(20.5%)and the control group 2(24.6%),the difference was statistically significant(P < 0.05),and there was no statistical difference between the control group 1 and the control group 2(P > 0.05);(2)Comparison of complication rate: the complication rates of control group 1,control group 2 and test group were 5.4%,7.9% and 1.8% respectively.There was significant difference between the test group and control group 2(P>0.05)but no significant difference among other groups(P > 0.05).(3)There was no death in the three groups(P > 0.05).conclusion: 1 Emotional therapy combined with Jiangqi Dayu decoction can effectively improve anxiety symptoms in patients with acute myocardial infarction of liver qi stagnation type and / or anxiety after PCI.2 Emotional therapy combined with Jiangqi Dayu decoction can effectively alleviate the symptoms of angina pectoris in patients with liver qi stagnation type acute myocardial infarction and / or anxiety after PCI,reduce the rehospitalization rate,and improve the quality of life and prognosis of patients. |
