| Background:Psoriasis is an immune-mediated,chronic,relapsing inflammatory disease triggered by a combination of environmental and genetic factors,typically presenting as scaly erythematous plaques or plaques,limited or widespread,non-infectious,persistent and often lifelong.Scalp psoriasis has a high prevalence rate and is typically the first site of onset.Scalp psoriasis is unique in that it has structures such as hair,which not only hinders the removal of scales,but also makes it difficult for topical medication to reach the scalp lesions,resulting in low bioavailability of the medication and reducing the therapeutic effect.Furthermore,the appearance of scalp psoriasis is easy to see,and severe cases are often accompanied by a large amount of flaking,which has a serious impact on patients’ social and daily life.97%of patients say that it affects their daily life,while more than 70%of patients have varying degrees of itching,causing varying degrees of tension,anxiety,depression,low self-esteem and other emotions.The mentor inherited the treatment ideas of Yanjing Zhao’s school of dermatology and combined with his own clinical experience to conclude that chubai lotion is effective in clinical treatment,but lacks specific data support and needs further research in order to promote it.Objective:To observe the clinical efficacy of chubai lotion in the treatment of scalp psoriasis(blood-heat evidence and blood-dry evidence).Methods:Sixty-six patients with scalp psoriasis who met the inclusion criteria and attended the outpatient clinic of the Department of Dermatology of Beijing Hospital of Traditional Chinese Medicine were randomly divided into 33 cases each in the treatment group and the control group.The treatment group was treated with chubai lotion and the control group was treated with selenium disulfide lotion.General data such as gender,age,duration of disease,allergy history,family history and the relationship between the onset of scalp lesions and other skin lesions were collected from all patients before treatment,and the lesions were observed and evaluated before treatment,after 2 weeks of treatment and after 4 weeks of treatment.The individual skin lesion scores,S-mPASI scores and pruritus scores were recorded before,after 2 weeks of treatment and after 4 weeks of treatment,the patients’subjective perception of drug use after 2 weeks of treatment and after 4 weeks of treatment,the patients’ quality of life(Scalpdex)scale before and after 4 weeks of treatment,and the presence or absence of adverse effects after treatment.Statistical data were then compared and analysed by SPSS26.0 software to evaluate the efficacy and safety of chubai lotion in the treatment of scalp psoriasis in both groups in terms of general information and clinical data.Results:A total of 61 patients completed the trial,31 in the treatment group and 30 in the control group,with a total of 5 cases shed.By comparing the general data such as gender,age,disease duration and family history between the two groups there were no statistical differences and were clinically comparable.By comparing the clinical data,there was no statistical difference between the two groups at baseline in the individual lesion scores(erythema,scaling,infiltration),pruritus score,Quality of Life Scale score and three dimensions of the Quality of Life Scale(symptoms,emotion,function)score(P>0.05),which were clinically comparable;the difference between the two groups in the lesion area score and S-mPASI score was statistically significant(P<0.05).The overall effect of the treatment group was better than that of the control group.There were no adverse reactions in the treatment group and one patient in the control group experienced scalp irritation and discomfort.Conclusion:Chubai lotion is safe and effective in the treatment of scalp psoriasis(blood heat and blood dryness),and can effectively improve the scalp lesions and quality of life of patients. |
