Objective:The purpose of this study is to observe the therapeutic effect and safety of Yueju pill on the syndrome of stagnated phlegm and qi in Graves disease through clinical case study, and to explore the practical value of this prescription in the treatment of Graves disease with traditional Chinese medicine, with a view to providing new theoretical basis, diagnosis and treatment ideas and treatment methods for clinical use of traditional Chinese medicine.Methods:60 cases of Graves disease with phlegm stagnation syndrome were divided into two groups by random sampling and grouping,30 cases in the treatment group and 30 cases in the control group. The control group was treated with methimazole tablets, while the treatment group was treated with traditional Chinese medicine(Yueju pill)on the basis of the control group. The total observation period was 8 weeks.Ob serve and compare the changes of FT3,FT4,TSH,TRAb,vascular endothelial growth factor, clinical symptoms and curative effect score of traditional Chinese medicine before and after treatment. Then, statistical analysis was carried out by SPSS26.0 statistical software to objectively evaluate the therapeutic effect and safety of Yueju pill on the syndrome of phlegm stagnation in Graves disease.Results:① Comparison of thyroid function:There was no difference in thyroid function between the two groups before treatment, which was comparable(P>0.05).The difference of thyroid function between groups after treatment was statistically significant(P<0.05);The results of intra-group comparison between the control group and the treatment group before and after treatment showed that the difference was statistically significant(P<0.05).②Comparison of vascular endothelial growth factor:There was no difference in the level of vascular endothelial growth factor between the two groups before treatment, which was comparable(P>0.05).After treatment, the difference of VEGF between the two groups was statistically significant(P>0.05),and the difference was statistically signi fi cant(P<0.05).③Comparison of TCM single symptom scores:There was no difference between the two groups in TCM single symptom scores before treatment, which was comparable(P>0.05).After treatment, there were statistically significant differences between the two groups in symptoms such as neck swelling, chest and hypochondriac pain, emotional distress, chest tightness,etc.(P<0.05),but there was no statistically significant difference in dry mouth and poor sleep(P>0.05).The intra-group comparison between the two groups before and after treatment showed statistically significant differences in the expression of anterior cervical swelling(P<0.05).The remaining symptoms of the control group showed no statistically significant differences before and after treatment(P>0.05).The symptoms of chest and hypochondriac pain, emotional distress, chest tightness in the treatment group showed statistically significant differences before and after treatment(P<0.05),while the differences of dry mouth and poor sleep were not statistically signifi cant(P>0.05).④Comparison of the total score of TCM syndromes:There was no difference between the two groups before treatment, which was comparable(P>0.05).Both the treatment group and the control group can significantly reduce the TCM syndrome score of patients(P<0.05).The effect of reducing TCM syndrome integral in the treatment group was better than that in the control group.⑤Comparison of curative effects of TCM syndromes:the total effective rate of the treatment group(86%)was significantly higher than that of the control group(4 8%),with a statistically significant difference(P<0.05).⑥Comparison of clinical efficacy:the total effective rate of the treatment group(93%)was higher than that of the control group(86%),with a statistically significant difference(P<0.05).⑦Safety and adverse reactions:blood routine test, liver and kidney function in the treatment group and control group were normal before and after treatment. One patient had loose stools after taking Chinese medicine, and one patient felt cold in the stomach and reduced appetite due to taking Chinese medicine. Conclusion:The modified Yueju pill can improve the thyroid function of GD patients with phlegm stagnation, reduce the level of vascular endothelial growth factor, and improve the symptoms of anterior neck swelling, chest tightness, dysphoria, chest and hypochondriac pain in GD patients. The medical and overall efficacy is better than the simple MMI treatment, and it is safe and reliable, without causing adverse reactions such as blood routine test, liver and kidney function damage, which is worthy of recognition in clinical application.
