Objective: To observe the clinical effect of ultrasy-guided injection combined with electroacupuncture in the treatment of shoulder pain after stroke,and to clarify the clinical advantages of ultrasy-guided injection combined with electroacupuncture in the treatment of shoulder pain after stroke,so as to provide new clinical ideas for the treatment of the disease.Methods: In this study,72 patients with typical post-stroke shoulder pain were selected and randomly divided into group A,group B and group C.Groups A and B were taken as control group,and group C was taken as observation group.There were 24 cases in each group.All three groups were given post-stroke basic treatment.Group A was treated with electroacupuncture for post-stroke shoulder pain,group B was treated with ultrasound-guided injection,and group C was treated with ultrasound-guided injection combined with electroacupuncture.Electroacupuncture takes 2 weeks as a course of treatment,a total of 1 course of treatment,ultrasound guided injection takes 1 time as a course of treatment,a total of 1 course of treatment.The improvement of upper limb function was observed by Fugl-Meyer evaluation scale before treatment,2 days after treatment and 4 weeks after treatment.Visual analogue scale(VAS)was used to evaluate the pain changes of patients before and after treatment.Barthel index(BI)was used to evaluate the effect of upper limb function on the ability of daily living.A total of three times were assessed,and adverse reactions were recorded during the study,and the clinical efficacy was comprehensively evaluated after treatment.Results :(1)VAS scores of the three groups were compared before and after treatment for three times,and VAS scores of the three groups were decreased.After 2 days of treatment,VAS scores of group C and group B were significantly decreased P<0.05;and the difference was statistically significant.VAS score at 4 weeks of treatment showed that group C was significantly lower than group B and group A,and the difference was statistically significant P<0.05;This indicates that the clinical treatment effect of group c in reducing VAS score is more significant.After three times of comparison of VAS scores among the three groups,there was improvement,but group C was better than group A and group B;(2)After three FMA scores before and after treatment,FMA scores in the three groups were improved,but FMA scores in group C were significantly improved,P<0.05 and the difference was statistically significant;After three FMA scores were compared between the three groups before and after treatment,the FMA scores of the three groups were improved,but the score of group C was significantly higher than that of group A and group B,and the difference was statistically significant P<0.05;This indicates that the clinical treatment effect of group C is more significant in improving the upper limb function of patients;(3)After three times of MBI score comparison before and after treatment,the MBI score in the three groups was improved,but the MBI score in group C was significantly improved,P<0.05 and the difference was statistically significant;After three times of MBI score comparison between the three groups before and after treatment,the MBI score of the three groups was improved,but the score of group C was significantly higher than that of group A and group B,and the difference was statistically significant P<0.05;This indicates that the clinical treatment effect of group C is more significant in improving the upper limb function of patients;(4)Comparison of efficacy: Comparison of curative effect of the three groups after treatment,the curative effect of group C is significantly better than that of group A and group B,and P<0.05;There were 23 cases in group A,and the total effective rate was 73.91%.The total effective rate of group B was 52.63%.There were 18 cases in group C,and the total effective rate was94.44%.Conclusion :(1)Electroacupuncture combined with ultrasound-guided precision injection can reduce the pain of the shoulder joint after stroke and improve the function of the shoulder joint.From the overall curative effect,this therapy can improve the function of the shoulder joint and is worthy of clinical promotion and application.All the three treatments could reduce VAS score,improve upper limb function and enhance daily living ability.Through clinical control studies,it is found that combined application is superior to single therapy,and its clinical efficacy is significant,the cure rate is high,the safety is high,the maneuverability is strong,and it has a synergistic effect in the treatment of post-stroke shoulder pain. |
