Objective:To summarize the academic thoughts and clinical experience of Professor Binfang Zeng; Taking the example of chronic hepatitis, by adopting the sharing platform of the clinic scientific research information, to establish the medical case database and knowledge base for the chronic hepatitis; To explore the clinic experience of treating the chronic hepatitis of Professor Binfang Zeng. Methods:To mainly focus on the inherited mode research of the Chinese tradition medicine, through the 3 years’ study with the teacher, to record the professor’s clinic medical bases in details; To analyze the professor’s clinic critical thinking; To summarize the professor’s drug use regularity; To use the methods of point mutual information and the data mining methods, like the complex network system to explore and research the implicit knowledge, such as the dialectical experience, compatible characteristics and drug usage habits.Results:(1)Based on the interrelationship between generation and restriction of five internal organs, Professor Binfang Zeng treats the chronic hepatitis that complies with the alternation of four seasons and points out the reinforcing and reducing to the five internal organs which means reinforcing mother viscera for treating deficiency of child viscera and purging child viscera for treating excess of mother viscera, prescription in the treatment of liver disease, flavoring spleen, inter-promotion of the lung to help kidney, renal water is fine for nourishing the liver; Follow the feature of liver belonging to Yin in physique and Yang in function, advocating the method of along the nature for reinforcement and against the nature for reduction.(2) It is put forward in the liver disease identification and treatment that improving the occasion, truncated therapy and eight methods for identifying and treating jaundice;Simultaneous regulation of liver, spleen and kidney, simultaneous treatment of phlegm, stasis and stagnation for identifying and treating liver-qi stasis. It is put forward in the syndrome differentiation of spleen and stomach disease that acrid opening, bitter downbearing and sweet reinforcement for identifying and treating stomach deficiency the new treating stomach; Treating constipation with four methods based on the internal organs and ascending lucidity and descending turbidity for identifying and treating diarrhea.(3)The most common symptoms of the 277 cases of chrome hepatitis B collected in this study include fatigue and the lack of energy, abdominal distension, pain in thorax, edema of both lower limbs. Chronic hepatitis B have 10 syndromes in total, the 5 most common are liver stagnation and spleen deficiency, interior retention of damp-heat, dampness-heat of liver and gallbladder, liver Qi stagnation as well as Qi stagnation and blood stasis, the patients who have such five syndromes total at 224, accounting for 80.87% of all patients. There are 12 therapies with frequency greater than 5 times or more, promoting blood circulation and nourishing liver and spleen, clearing heat and expelling dampness, tonifying liver and kidney, regulating Qi and eliminating blood stagnation, propelling blood flow and invigorating spleen are the most common treatment methods. (4) Based on node degrees of drugs commonly used to treat chronic hepatitis B, Xiao Chaihu Decoction and Siwu Decoction can be used as the basic formula, the Chinese traditional medicines with cold, warm and mild properties, and those with sweet, bitter and spicy flavors are used to treat chronic hepatitis B; The treatment of treat chronic hepatitis B focuses on the dredging of spleen channel, lung channel and liver channel. (5)Point mutual information method to analyze symptom-syndrome relationship:the main symptoms and syndrome of liver stagnation and spleen deficiency include fatigue, the lack of energy, dry mouth, dull pain in right lateral thorax, anorexia and abdominal distention; Symptom-treatment relationship:improving blood circulation, nourishing liver and tonifying spleen are the main treatment methods of liver stagnation and spleen deficiency; Symptom-drug relationship:radix bupleuri, codonopsis, radix scutellariae, angelica, radix paeoniae rubra, radix glycyrrhizae preparata, rhizoma Pinellinae Praeparata, radix rehmanniae, cyperus rotundus, etc.; Complex network analysis:Xiao Chaihu decoction combined with Siwu Decoction is the major formula to improve liver stagnation and spleen deficiency. The main symptoms of syndrome of interior retention of damp-heat include abdominal distention, fatigue and the lack of energy, dry mouth, yellow urine, dizziness, yellow eyes, edema on both lower limbs and discomfort in right lateral thorax, etc.; Symptom-treatment relationship: clearing heat, removing dampness and eliminating phlegm are the treatment methods of interior retention of damp-heat. Symptom-drug relationship:capillary wormwood herb, wine-treated rhubarb, polyporus, alisma orientalis, atractylodes, rhizoma atractylodis, dried tangerine or orange peel, amomum villosum, mirabilite and alum, etc.; Symptom-formula relationship:Yinchen Siling powder is the main formula of treating interior retention of damp-heat. The major symptoms of syndromes of liver Qi stagnation include fatigue and the lack of energy, dry mouth, periumbilical pain, anorexia, pain in thorax, etc.; Symptom-treatment relationship:promoting blood circulation and nourishing liver are the treatment methods of liver Qi stagnation; Symptom-drug relationship:angelica, radix bupleuri, Ligusticum wallichii, radix glycyrrhizae preparata, hawthorn, radix paeoniae alba, rhizoma Pinellinae Praeparata, Scutellaria baicalensis, cyperus rotundus, radix scutellariae, radix rehmanniae, etc.; Symptom-formula relationship:Chaihu Shugan Powder is the basic formula to treat liver Qi stagnation. The main symptoms of syndrome of dampness-heat disease of liver and gallbladder include pain in lateral thorax, fatigue and the lack of energy, dry mouth, nausea and vomiting, anorexia, bitter mouth, etc.;Symptom-treatment relationship:dampness-heat disease of liver and gallbladder should be treated by clearing heat and expelling dampness in liver and gallbladder; symptom-drug relationship:capillary wormwood herb, tuckahoe, alisma orientalis, polyporus, angelica, dried tangerine or orange peel, peach kernel, etc.; Symptom-formula relationship:Yinchen Siling Powder is the basic formula of treating dampness-heat disease of liver and gallbladder. The main symptoms of syndrome of Qi stagnation and blood stasis include fatigue and the lack of energy, dry mouth, palpitation, dull complexion, etc.; Symptom-treatment relationships:Qi stagnation and blood stasis can be cured by regulating the flow of Qi, propelling blood circulation and nourishing spleen; Symptom-drug relationship:radix curcumae, radix paeoniae alba, radix paeoniae rubra, codonopsis pilosula, bupleurum, nutgrass galingale rhizome, fructus aurantii, salvia miltiorrhiza, etc.; Symptom-formula relationship:Sini Powerder is basic formula of treating Qi stagnation and blood stasis. (6) Complex network analysis:basic and core formula of treating chronic hepatitis B put forward by professor Zeng Binfang:taking Xiao Chaihu decoction and Siwu Decoction as basic drugs, accompanied by Chen Siling Powder and linearstripe rabdosia herb to clean heat and expel dampness; Oldenlandia diffusa and rhizome of manyleaf paris to expel heat and remove toxicity; Cyperus rotundus, radix curcumae, radix aucklandiae, fructus aurantii and malt to sooth liver, regulate Qi and promote blood circulation. Astragalus, Chinese yam, amomum villosum, semen coicis, dried tangerine or orange peel, magnolia officinalis to nourish spleen and clear away dampness; Turtle shell, tortoise shell, pangolin scales, fructus lycii, rhizoma polygonati, fructus ligustri lucidi to nourish Yin and liver as well as eliminating masses. As "kidney and liver are from the same origin", and "blood and essence are derived from the same origin", "nourishing kidney can tonify kidney; Crataegus pinnatifida bunge, schisandra chinensis, Semen Ziziphi spinosae can supply nutrients to internal organs; Raw oysters, thunberg fritillary bulb, wine-treated rhubarb, mirabilite and alumto can soften hard lump. All the ingredients can jointly propel blood circulation, nourish liver and tonify spleen, clear heat, expel dampness and eliminate toxicity, nourish Yi and soften hard lump. The formula of treating chronic hepatitis B developed by professor Zeng Binfang fully reflects the treatment principle of "promoting blood circulation, nourishing liver and tonifying spleen", and also confirms that chronic hepatitis B is caused by "poor blood circulation and insufficient nutrient supply to liver.Conclusion:(1)Professor Zeng Binfang advocates that "taking preventative measures before getting disease"; And puts forward that "the root cause of internal organ disorders is the poor function of stomach and spleen"; And adheres to the idea that "sound flow of Qi and blood expels all maladies" and almost all illness are caused by "blood and Qi stagnation"; "The illness shall be cured radically (based on Yin-Yang theory and stomach-qi leading to the heart)". The dialectic thought and treatment features of Professor Zeng Binfang:exuberance and debilitation of pathogenic Qi or healthy Qi, focusing on the unity of Yin and Yang, diagnosing diseases based on eight-principle syndrome differentiation; Promotion, restriction, over-restriction and reverse restriction in five elements, with adopting viscera syndrome differentiation to find disease location; Detecting the root cause of illness and grasping major pathogenesis, as well as adopting reinforcing and reducing methods to treat disease. (2)Professor Zeng Binfang used to put forward liver reinforcing and reducing methods: interior organ Qi corresponding to nature rhythms as well as interpromotion and restraint of the five elements. (3)Professor Zeng Binfang pays attention to the interaction of interior organs in the treatment of hepatitis B, holding that "poor Qi and blood circulation results in liver disease, which causes spleen disease", and treats disease by "promoting blood circulation and nourishing liver and spleen"; Focuses on the improvement of the functions of interior organs, and adopts reinforcing and reducing methods to treat disease; She also advocates "theory of lingering toxins", combines Chinese traditional medicine and western medicine, and focuses on detoxification throughout the whole treatment process.
