Oral carcinoma is the most common malignant tumor in head and neck region. Oral squamous cell carcinoma is about 80 per cent in oral carcinoma. If there is cervical lymph node metastasis, it will affect the patient prognoses. Although many people know it, its related molecular mechanism of lymph node metastasis is still unclear. It is highlight and difficulty in tumor research. It is also a main obstruct in overcoming tumor. New approaches in elucidating the lymph node metastasis mechanism of oral squamous cell carcinoma are thus urgently needed.There are many steps during malignant tumor cell metastasis. It include four steps: 1. Dissolve the basal membrane and invade into around tissue. 2.Dissolve-7-the wall of lymph vessel and enter into the duct. 3. Move to lymph node. 4.Form a metastasis tumor in lymph node. Every step is controlled by gene. The major are dissolving the membrane and entering into new lymph vessel. According to the recent research Matrix metalloproteinase 2 is a major enzyme. Lyphmangiogenesis is related to VEGF-C gene. But the exact mechanism is now not very clear and there are many viewpoints and many debates in it. Furthermore the related research is focus in mammary carcinoma and digestive tract carcinoma.There is still no report on lymph node micrometastasis of oral carcinoma. This study is to detect lymph node micrometastasis in cervical with the cytokeratin protein expression. It make the result more scientific and more persuading. These dates would be of value to clarify the related mechanism and provide new approach in treatment the lymph node metastasis. There should be a very extensive clinical meaningSection 1The study on related mechanism of lymphangiogenesis in oral squamous cellcarcinoma15 cases of normal oral mucous membrane 47 cases of oral squamous cell carcinoma and 6 cases of transplant tumor were used in this study. After be done of frozen section,5'-nucleotidase was detected in microlymphatic vessel with enzyme-histochemical reaction. Microlymphatic vessel density in normal tissue and tumor was determinted by it. We also assessed the mRNA expression of VEGF-C VEGFR3 INOS in normal mucous membrane > oral squamous cell-8-carcinoma and Tca8113 cell line. We analysed the relationship between three gene expression and Microlymphatic vessel density (MLVD).The results showed that microlymphatic vessel density was higher in tumor than normal tissue. There was a significant difference between them. The percentages of tumors with higher expression compared to the mean values of normal tissues were 57.4% for VEGF-C 61.7% for VEGFR3 68.1% for iNOS. Significant positive relationship were found between VEGF-C and MLVD, VEGFR3 and MLVD, iNOS and MLVD, VEGF-C and VEGFR3, VEGF-C and inos, VEGFR3 and iNOS.It indicated that VEGF-C > VEGFR3, INOS play an important role in lymphangiogenesis. VEGF-C^ VEGFR3 - INOS stimulate lymphangiogenesis around the tumor in autocrine and paracrine way. It provided the histological base for lymph node metastasis. Tumor cell express high VEGF-C and induce the lymphatic vessel endothelial cell highly expressed VEGFR3, then led iNOS higher expression in endothelial cell, at last result in high density of lymphatic vessel. Three gene expression were important molecular event in lymphangiogenesis around the tumor.Section 2Study on the relationship between lymphangiogenesis and cervical lymphnode micrometastasisAccording to the first section result, the microlymphatic vessel density was higher in tumor because of VEGF-C VEGFR3 iNOS high expression.
