[Objective] To investigate the relationship between severity of acute pancreatitis and apoptosis of acinar cells. [Methods] Using intrapancreatobiliary duct injection of different doses sodium deoxycholate, the acute edematous pancreatitis, acute moderate necrotic pancreatitis, and acute severe necrotic pancreatitis model were established in Wistar rats. The apoptotic ratio of acinar cell was then determined by using methods of in situ end labeling (TUNEL), and the pancreatic wet weight, volume of ascites, blood amylase content, and pancreatic histological activity index (HAI) were determined.[Results] There were no or little apoptotic acinar cells in acute simple edematous pancreatitis model. But there were apoptotic cells in acute hemorrhagic and necrotic pancreatitis model. With the increase of hemorrhagic and necrotic degree, the apoptotic ratio of acinar cell first increased rapidly and then decreased gradually, meanwhile the ratio of necrotic acinar cells increased continuously.[Conclusions] In acute simple edematous pancreatitis, there were no or very little apoptotic acinar cells. But in acute moderate hemorrhagic and necrotic pancreatitis, the apoptotic ratio of pancreatic acinar cells was correlated positively with the severity of pancreatitis. While in acute severe hemorrhagic and necrotic pancreatitis, the apoptotic ratio of pancreatic acinar cells was correlated negatively with the severity of pancreatitis. It is suggested that acinar cell apoptosis maybe one of the mechanisms of self-defense.
