Objectives To study the local immune response and the clinical curative effect of injecting the highly agglutinative staphylococin(HAS) as superantigen into the tumor areas of the liver cancer patients combing with the percutaneous microwave coagulation therapy(PMCT) under the ultrasound guide and to evaluate if the local immune response has influence on prognosis of patients.Materials and Methods All the 92 cases of liver cancer patients were approved by pathology. 47 of them were treated by PMCT combing with percutanous Highly Agglutinative Therapy (PHIT) under ultrasound guide and 45 were treated only by PMCT. The patients from the group of PMCT combing with PHIT were given HAS through intramuscular injection experimentally for 3 days from the third week after the treatment of PMCT. And after identified no side effects, the patients were given injection of HAS 2000 units into the local tumor areas in the third and seventh day and the first and third and sixth month after intramuscular injection of HAS. At the time of the fourth week after PMCT and before PMCT and PHIT, the patients were given percutaneous biopsy under ultrasound guide. And the samples were stained by HE and immunohistochemical methods. Moreover the samples were scaned and the function of immunocytes were observed by electronic microscope after HAS was injected into the local tumor areas.Then the infiltration of CD3+, CD4+, CD8+, CD57+and CD68+of local treatment areas were compared before and after the treatment in the group PMCT and the group PMCT combing with PHIT. We also compared it between the two groups.. Moreover the infiltration changes of IL-2, IL-6 TNF-a of local tumor areas before and after treatment of PHIT were compared . Meanwhile, 39 cases of PHIT patients and 39 PMCT patients with hepatocellular carcinoma were made follow-up surveys of survival and recrudescence. And the survival rate and unrecrudescence rate were compared between the PMCT and the PHIT. Moreover, It were analyzed that the relation between the infiltration of immunocytes and survival as while as recrudescence.Results (1) Group PMCT: pre- PMCT, the immunocytes such as CD3\ CD4\CD8\CD57+ and CD68+ infiltrated in the tumor and adjacent hepatic tissue more or less. The degree of infiltration of CD3\ CD4+, CD8\ CD57+ and CD68+ increased significantly in post-PMCT as compared with those of pre-PMCT(P<0.05) and the immunocytes infiltrated mainly in the outer fringe of the necroses and adjacent hepatic tissue. Group PHIT: after injection of HAS into the local tumor areas, the infiltration of CD3+, CD4+, CD57+ and CD68+ increased more or less compared with those of pre-PMCT and pre-PHIT (P<0.05). And they lasted to 1 month after intramuscular injection of HAS (P<0.05). The increased immunocytes were functioning powerfully after PHIT. (2) The infiltration of CD3+,CD4+, CD57+ and CD68+ in group PHIT increased significantly compared with that of group PMCT (P<.05) . (3) After local injection of HAS, the localpositive infiltration of IL-2 increased significantly than that of pre-PMCT (P<0.05) . (4) The survival rate of group PHIT and group PMCT in the first, second, third, fourth year were 93.63%, 83.04%, 74.14%,61.78% and 88.89%, 80.81%, 65.69%, 46.99% respectively. And the unrecrudescence rate were 90.00%, 68.96%, 56.42%, 56.42% and 77.26%, 51.24%, 46.97%, 46.97%. There were not significant differences between two groups (P>0.05). However, the short-time unrecrudescence rate in group PHIT increased significantly than that of group PMCT in 19 cases respectively (P<0.05). (5) Before PMCT, the positive express of CD3+, CD4+ and CD57+ in survival group and unrecrudescence group were higher than those of dead group and recrudescence group(P<0.05). After PMCT, the patients in unrecrudescence group had higher infiltration of positive immunocytes than those of patients in recrudescence group (P<0.05).
