A Study On The Mechanisms Of Inhibition Of Shen'an Extraction To The Renal Fibrosis In The Remnant Kidney Model

Astragalin, Icariine and Anthraquinone derivatives were respectively extracted from Radix Astragali, Herba Epimedii and Radix Et Rhizoma Rhei which have been traditionally prescribed for the treatment of progressive renal diseases for a long time in HuBei Hospital of Traditional Chinese Medicine. In the present study, the combination of Astragalin, Icariine and Anthraquinone (Shen'an Extraction) was studied to demonstrate the pharmic, biochemical, histological and cellular effects and mechanisms on remnant kindney (RK) model.The technics and quality standards of Shen'an Extraction was established by the pharmic study. The technic that Radix Astragali was extracted twice for one hour by eight times of water could get no less than 2.5mg of Astragalin per 10g Shen'an originals. The method that combination of Herba Epimedii and Radix Et Rhizoma Rhei Epimedoside was extracted twice for one hour by 10 times of 70% ethanol could yield no less than 1.8mg of Icariine and 2.8mg of Anthraquinone per lOg Shen'an originals.In the biochemical researches, Wistar rats were distributed into seven groups. Group 1 was normal control, Group2 sham operation control, group3 model control and 4 to 7 treated groups. Group 2 underwent a sham operation by decapsulation of the kidney and group3 to 7 underwent a right subcapsular nephrectomy with surgical resection of the upper and lower thirds of the left kidney. Group 1 to 3 received distilled waterlO ml/kg body weight, group4 to 6 received Shen'an 2g/kg, 4g/kg, 8g/kg body weight, and group7 received Niaoduqing 3.6g/kg body weight once daily for 8 weeks respectively. At the start of the protocol, half a month, one month and two months after administration, body weights and livability were determined, blood samples were obtained for BUN, Scr, ALB, TP, Hb, RBC, Ca, P, SOD, NO evaluation.It was found that all of Shen'an treated groups resulted in improved body weights, ALB, Hb, RBC at one month point (versus model P<0.05, P<0.01), and more significant results at two month point (P<0.05, P<0.01). Shen'an 4g/kg, 8g/kg yielded better effects on Hb and RBC than Niaoguqing (P<0.05) and a positive relationship with quantity and effects(r = 1, r = 0.969). At one month, SOD in all of She'an treated groups was evidently higher, NO was lower than model (P<0.05, P<0.0l). At two month, Shen'an 4g/kg, 8g/kg got a higher Ca and a lower P than model (P<0.05> P<0.01). All of Shen'an treated groups resulted in improved BUN and Scr (versus model P<0.05, P<0.01) and a positive relationship with quantity and effect( r = 0.999 ). Shen'an 4g/kg, 8g/kg had a more significant result in BUN than Niaoduqing (P<0.05). There for the results of functional study suggest that Shen'an Extraction could stabilize the renal function of CRF model through improving the nutrition and proteinmetabolism, balancing with Ca and P, alleviating NO and Oxidant injury, and lead to a higher livibility of CRF animals.(versus model P<0.05)In the histological researches, the seven groups of RK were collected from the functinal study at the time of sacrifice after Shen'an Extraction administration for two months. Histological and collagen IV(CoIV) semiquantitative analyses, proliferous cell nuclear antigen (PCNA) positive cells and total glomerular cells were determined.It was found that all of Shen'an treated groups showed significantly lower Co IV integral than model (P<0.05, P<0.01), and Shen'an 8g/kg was more significant than Niaoduqing (P<0.05). PCNA positive cells and total glomerular cells in all of Shen'an treated groups were evidently decreased than model (P<0.05, P<0.01), and total glomerular cells in Shen'an 8g/kg was more evident than Niaoduqing (P<0.05). All of Shen'an treated groups showed lower histological semiquantitative integral than model (P<0.05, P<0.01), Shen'an 8g/kg was more evident than Niaoduqing (P<0.01). There for, the results of histologiical study suggest that Shen'an could prevent renal fibrosis through inhibiting the proliferation of PCNA positive cells and total glomerular cells, decreas...
