High intensity focused ultrasound (HIFU) has developed fast in USA, GB, Japan and China since the 80's of the 20th century. In HIFU technology, extracorporeal low power ultrasound beams, which may be focused on the targeted tissue in depth in human body, are utilized. The focused high intensity ultrasonic beams cover and destroy the targeted tissue with transient hypothermia and cavitation, resulting in coagulative necrosis of the target without damaging tissue in vicinity. Hypothermia refers to temperature rising within the targeted region reaching 60 – 100℃ in a few seconds. At present HIFU has been widely used in the clinical treatment of benign prostatic hypertrophy, hepatocarcinoma, bone tumor, breast cancer and soft tissue sarcoma et al. However, study on HIFU in the treatment of some benign tumor such as myoma of uterus has not carried out widely. HIFU technology is considered as a focused ultrasound surgery (FUS). The characteristics of HIFU is exact ablation of lesion without intruding into tissue. It goes with tide of invasive operation developing to noninvasive operation. This study has further worked on HIFU in the treatment of human uterine fibroids and uterine adenomyoma on the basis of study did by Dr.Chang Shufang and Dr. Xiong Zhengai.1. To explore primarily dosage regularity of high intensity focused ultrasound (HIFU) in the exposure of human uterine fibroids ex vivo:total uterine samples after operation were divided into several groups randomly and irradiated by different ultrasound sound power. To compared volum and B-mode ultrasound gray changes et al. between different groups. The results included: ①When irradiation time did not change, volum of targeted area and change of gray value would increase along with rising of sound power;②When sound power did not change, volum of targeted area and change of gray value would increase along with rising of irradiation time. However, when dosage came to a certain value, no matter how irradiation time and sound power arose, increasing of volum of targeted area and gray value under B-mode ultrasound went slowly. The best out dosage was 120W×100s;③Irradiation sound power effected more obviously than irradiation time on changes of targeted volum and gray value under B-mode ultrasound.2. To explore cell effect of high intensity focused ultrasound(HIFU)on uterine fibroids before cutting off blood: JC-mode high intensity focused ultrasound (HIFU) therapeutic system (developed and produced by Chongqing Haifu Technology Co.Ltd.) was performed on thirty patients with uterine fibroids who had been ready to receive total hysterectomy earlier before. All the patients agreed with HIFU treatment. After routine entering into abdomen, HIFU knife was used to scan in a straight line on fibroids immediately. After hysterectomy, some tissue in irradiated area was taken out and examined by TTC stain, light and electron microscope observation, and DNA EP experiment. The results demonstrated that the irradiated tissue from thirty samples became hard and white, TTC stain was white and showed clear boundary between targeted tissue and border tissue, light and electron microscope demonstrated coagulative necrosis of fibroids cell, DNA displayed a scattered figure with shape of slice. All above results showed HIFU had a certain killing effect on human uterine fibroids cell in vivo, which provided a powerful theoretical foundation for HIFU in the treatment of human uterine fibroids.3. To explore the safety of HIFU irradiation on human uterine fibroids in vivo:to comprehend the structure and function of border mucous membrane and myometrium cell according to the compare some indexes such as morphological manifestation, cellular recipient expression, NADH-diaphorase stain of mucous membrane and myometrium closed to HIFU irradiated area with that of mucous membrane and myometrium not irradiated by HIFU.
