Endometriosis (EM) is very common and difficult in gynecologic diseases, and the incidence of it has increased recently. EM is the main reason of female-associated infertility. Minority of endometriotic cysts may be malignant . The life quality of women is threatened by it.Salvia?chinensis is a Chinese herb which can improve blood circulation and relieve stasis. Firstly we studied on the effects of Salvia ?chinensis on EM. Rat models of EM were made and pathological , immunopathological and molecular biological methods were used to discuss the pathogenesis of EM and therapeutic mechanism of Salvia?chinensis? on the basis of observation on the evidences of histology, endocrinology, invasion mechanism, apoptosis and its correlated genes. Conclusion:1 .Established endometriosis (EM)models successfullyEM rats models were established. 50 Wister rats' endometria were autotransplanted . 66% of models were established successfully. Adenoepithelium and mesenchymal cells were seen, which kept original histological characteristics, and the models were reliable. 2 Inhibition of Salvia? chinensis? to EM Inhibition were shown by observing focal size, histological and pathological changes. The growth of heterotopic endometria were inhibited and the focal size were reduced after treatment with Salvia?chinensis . The growth of heterotopic endometria were inhibited in ultrastructure. By electron microscopy, there were many morphological characteristics of apoptosis including compaction and margination of nuclear chromatin, nuclear fragments and apoptotic bodies. We could draw a conclusion that Salvia?chinensis? has a prominent role to induce EM cells apoptosis .3 Effect on endocrine function of Salvia?chinensis? in EM modelsBecause EM is an estrogen-depended disease, changes of endocrine function have a deep effect on prognosis. The experiment showed that Salvia?chinensis?could reduce FSH, LH and E2 levels slightly by RIA method compared to that of Alarelin -treated rats. The stable endocrine changes were still observed through vaginal smears in Salvia?chinensis-treated rats. Therefore, Salvia?chinensis?not only had a role to inhibit growth of heterotopic endometria though low level estrogens, but also prevented side effects by hypoestrogenemia. Salvia?chinensis? has a promising value on EM treatment.4 Influences of Salvia?chinensis? on adhesion, invasion and vessels formation in EMAs we known adhesion, implantation and growing in abdomen is the key cause in pathogenesis of heterotopic endometrium. Immunohistochemical staining methods were used to test E-Cad, MMP-2 and VEGF expressions in local tissues. The results indicated that the expression of E-Cad both in homotypic and heterotopic endometrium were lower than that of normal rats. We could draw a conclusion that cells depolymerized was the result of E-cad low expression during the process of EM leads to decrease in adhension function between endometrium epithelial cells , and between endometrium epithelial cells and matrix, so that it was easy for endometrium epithelial cells to dissociation from the normal location. Salvia?chinensis? and Alarelin could improve E-Cad expression in homotypic and heterotopic endometrium. Therefore Salvia?chinensis? could improve adhension ability of local tissue and reduced the incidence of EM invading and transferring.When endometria with poor ability of adhension countercurrent to abdomen, they must depolymerize extracellular matrix(ECM) of basilar membrane cells and other cells, so they can implant and grow. MMP-2 can depolymerize all ECM. The result of our experiment showed that high expression of MMP-2 in homotypic and heterotopic endometrium leaded to extracellular matrix(ECM) depolymerizing and destroying, so endometrium epithelial cells could invade , implant and grow. At the meantime it could destroy the circumstance matrix, enlarge the lesion size.
