Expression of Transforming Growth Factor B1 and Basic Fibroblast Growth Factor after Glaucoma Filtration Surgery in a Rabbit ModelPurpose: To discuss efficiency of the undegraded "M" type PMMA stent and feasibility of surgical manner through building trabeculectomy combined with this support implanting under the scleral flap(named filtering pathway plastic surgery) in a rabbit model; To determine whether PMMA stent combined with low-concentration and short-time mitomycin can improve the effect of this surgery, so as to giving further experimental basis for clinical applying; To gain a further understanding of the role of transforming growth factor B1 (TGF-B1)and basic fibroblast growth Factor(bFGF) in the scar formation after experimental glaucoma filtration surgery through measuring the expression of TGF- B1 and bFGF in the tissue of surgical field, and to evaluate the safety and histo-compatibility through observing the scar formation after PMMA stent implanted, at the same time making a primary study about interaction between mitomycin and TGF-B1, bFGF.Methods: Normal rabbits were randomly divided into experimental group for surgery: Filtering pathway plastic surgery was performed in left eye and trabeculectomy was performed in right eye in experimental group, the tissue of surgical field of two eyes of rabbits was obtained postoperatively days- 3,5, and 14; Both eyes of four rabbits that did not undergo surgery were used as the control. In addition, filtering pathway plastic surgery combined with mitomycin was performed in both eyes of two rabbits. 1) The changes of intraocular pressure(IOP) and filtering bleb preoperatively and postoperatively different time points were compared between filtering pathway plastic surgery and trabeculectomy, and the filtering pathway space changes were observed using ultrasound biomicroscopy(UBM); The changes of IOP and filtering bleb were observed in filtering pathway plastic surgery. 2) The expression of TGF- P 1 and bFGF in the tissue of surgical field at different time points after the two differenttypes of glaucoma filtration surgery was measured using reverse transcription polymerase chain reaction(RT-PCR); The expression of TGF-B1 and bFGF at days 7 after filtering pathway plastic surgery combined with mitomycin was measured. 3) Protein expression and cell locations of TGF-B1 and bFGF were determined in the tissue of surgical field at different time points after the two different types of glaucoma filtration surgery using immunohistochemistry.Results: 1) There was no statistically significant difference in IOP preoperatively and postoperatively days 14 between filtering pathway plastic surgery and trabeculectomy, and there was statistically significant difference in IOP postoperatively days 3 and 7. 2) There was greatly statistically significant difference in IOP at days 3, 7 and 14 after filtering pathway plastic surgery compared to IOP preoperatively (P<0.01). IOP decreased by 48.4% at days 3, 46.2% at days 7 and 23.8% at days 14; There was statistically significant difference in IOP postoperatively days 3, 7 compared to days 14 respectively, whereas there was no statistically significant difference in IOP between days 3 and days 14. 3) There was statistically significant difference in IOP at days 3, 7 and 14 after trabeculectomy compared to preoperatively. IOP decreased by 36.0% at days 3, 31.6% at days 7 and 16.5% at days 14; There was no statistically significant difference in IOP among days 3, days 7 and days 14. 4) There was no significant difference between filtering pathway plastic surgery and trabeculectomy in filtering blebs scope, which was between low and middle grade; Strong echoes of PMMA stent and clearly weak echoes of aqueous humor filtering space were shown at days 3 and 7 using UBM after filtering pathway plastic surgery, and still shown at days 14. Clearly weak echoes of aqueous humor filtering space were shown at days 3 after trabeculectomy, but almost no seen at days 7 and 14. 5) The levels of TGF-B1 and bFGF expression in the normal control...
