Study On The Relationship Among The Polymorphisms Of Estrogen Receptor Gene And Vitamin D Receptor Gene,Syndrome Differentiation Type Of Traditional Chinese Medicine And Bone Mineral Density In Primary Osteoporosis

Objective: To study on the relationship among the polymorphisms of estrogen receptor (ER) gene (Xba I Pvu) and vitamin receptor gene (Bsm I ) syndrome differentiation type of traditional Chinese medicine(TCM) and bone mineral density (BMD) in primary osteoporosis.Methods: The lumbar vertebrea, femur neck, trochanter and Ward's region BMD of 918 osteoporosis patients of the Han nationality in Fu Zhou city were measured by dual energy X-ray absorptiometry. The age menopause age the duration of menopause height body weight and body mass index(BMI) were noted. 543 patients were divided into Kidney deficiency type (Kidney Yin deficiency type Kidney Yang deficiency type Kidney Yin Yang double deficiency type) Spleen Kidney both deficiency type Liver Kidney both deficiency type single Spleen deficiency type> single Liver deficiency type and other types. In 256 patients of Kidney deficiency type,the polymorphisms of ER gene and VDR gene were detected respectively with the endonuclease Xba I Pvu and Bsm I through PCR-RFLP method. The results were analyzed with SPSS software.Results(1)In women patients, their age were older and their duration were longer, their BMD were lower. The BMD were droped very significantly after 60 ages in female. There was a danger of lower BMD among lower BMI female. The main factor affecting BMD in lumbar femur neck and Wards was age , and in trochanter were body weight. In male patients, the important factor affecting BMD was weight. Lower BMI was the danger of lower BMD. (2) Kidney deficiency was main reason of TCM theory in osteoporosis. Usually, Sjpleen deficiency and Liver deficiency went with Kidney deficiency. Kidney Yin deficiency were often happened in female, while Kidney Yang deficiency in men.There was a relationship between syndrome differentiation type of TCM and BMD in men, but no connection in female. (3) In Fu Zhou city, VDR gene types: the frequence of bb type was most, 78. 9% in female and 94. 1% in men. The frequence of Bb type was more. The frequence of BB type was fewest, 2.8% in female and no in men. The frequence of B allele was 2.9% in men and 11.9% in female, while the frequence of b allele was 97. 1% inmen and 88.1% in female. The frequences of B and b alleles were more different between men and female. In bb type women, BMD of lumbar was lower in Kidney Yin Yang double deficiency type than Kidney Yin deficiency type, and the factors affecting BMD were age, duration of menopause, height and body weight, but only age was the facter affecting BMD in Bb type.The Bsm I genotype VDR gene were nonassociated with BMD in female. (4) ER gene types:the frequence of XX, Xx, xx types were respectively 18. 5%, 66.8%, 14.7% in female, and 25. 7%, 51. 4%, 22.9% in men. The frequence of PP, Pp pp types were respectively 19.2%, 47%, 33.8% in female, and 16. 7% 52.8%, 30.6% in men. The Xba I , Pvu genotypes of ER gene were nonassociated with BMD in female. In Xx type female, BMD in Wards wa.s lower in Kidney Yin Yang double deficiency type than in Kidney Yin deficiency type. In Kidney Yin deficiency type, BMD in Wards was lower in PP type than in Pp type and pp type. In Kidney deficiency XX type female, age, the duration of menopause menopause age, body weight and BMI were related with BMD. In Kidney deficiency Xx type female, age, the duration of menopause and height were related with BMD. In Kidney deficiency xx type female,age the duration of menopause menopause age height, body weight and BMI were not related with BMD. In Kidney deficiency PP type female, body weight and BMI were related with BMD. In Kidney deficiency pp type female, age and height were related with BMD. In Kidney deficiency Pp type female, age, the duration of menopause, body weight and BMI were related with BMD. So combining ER genotype with syndrome differentiation type of TCM was better than only using syndrome differentiation type of TCM in filtering the factors of affection BMD.
