The First : The research of the clinical living-related small boweltransplantationObjective To report the first clinical living-related small bowel transplantation.Method An 18-year-old boy with short gut syndrome underwent the living-related small bowel transplantation, with the graft kaken from his father(44-year-old). A segment of 150cm distal small bowel was resected from the donor. The donor ilecolic artery and vein were anastomosed to the recipient infrarenal aorta and vena cava respectively. The intestinal continuity was restored with an end-to-end anastomosis between the recipient jejunum and donor ileum, and the distal end was fistulized. Posttransplant immunosuppression was initially administered with FK506, MMF and prednisone.Result The patient has better life for 6 years after living-related small bowel transplantation,and has a free diet, enable attend working.The absorbent function of the graft is normal for long time.Conclusion. It is a good way to perform the living-related small bowel transplantation for the patients of short bowel syndrome. The Second Evaluation of perioperative serum cytokine level in AcuteRejection diagnose of Human living-related small bowel transplantationAim To investigate the perioperative serum levels of IL-2R, IL-8 et al in the firstcase of living-related small bowel transplantation in China and to study theinterrelationship between the changes in serum IL-2R, IL-8 et al levels and theacute small bowel allograft rejection.Method Serum levels of IL-2, TNF-α, IL-2R, IL-4, IL-6, IL-8 were analyzed bya solid-phase double ligand ELISA method.Results IL-2, TNF-α, IL-2R, IL-8 serum levels increased at K 2 POD, It's thefirst peak level. The second peak level occurred at 67 POD with acute smallbowel allograft rejection, the acute small bowel allograft rejection was curedimmediately, the recipient had enjoied health for 19 month.Conclusion In the human living-related small bowel transplantation, there wereclose correlation between serum IL-2R, IL-8 level and the acute small bowelallograft rejection.
