Objectives1. To compare the expression of TRAIL and DR4, DR5 in INF-lymphocyte in graft jejunums of living-related small bowel transplan- tation in different periods of postoperation. 3. To explain the role of TRAIL/TRAILR system in rejection of living-related small bowel transplantationMethods1. To collect the biopsy specimens of graft jejunums of 4 clinical living-related small bowel transplant patients in different periods of postoperation in our hospital during 1999-2007. Divided the biopsy specimens into acute rejection group, chronic rejection group and no rejection group according to their clinical data and optical microscope diagnosis with or without rejection.2. To quantitatively measure the expression of TRAIL, DR4, DR5 with immunofluorescent staining and laser scanning confoeal microscope technique of INF-lymphocyte in graft jejunums of living-related small bowel transplantation.3. To summarize the correlation of rejection and the expression of TRAIL and DR4, DR5 of INF-lymphocytes in graft jejunums of living-related small bowel transplantation combined with the clinical medical recordsResults1. 20 specimens of graft jejunums of clinical living-related small bowel transplant patients in our hospital were collected including 8 acute rejection, 7 chronic rejection and 5 no rejection specimens.2. The expression of TRAIL of rejection groups (acute rejection group, chronic rejection group) was significantly higher than that of no rejection group. (P<0.01) There was no significant difference in acute rejection groups and chronic rejection groups(P>0.05). By immunofluorescent staining and laser scanning confoeal microscope technique we can see that the expression of TRAIL mainly on infiltrating T lymphocytes.3. DR4, DR5 of INF-T-lymphocytes were lowly expressed in acute rejection group than those of chronic rejection group and no rejection group (P<0.01). But there was no significant difference among other two groups.4. DR4 receptor of NK cells was low expressed in acute rejection and chronic rejection group. Significant difference has been found to compare with no rejection group (P<0.01). There was no significant difference between acute rejection and chronic rejection group (P>0.05). DR5 of NK cells were lowly expressed when acute rejection happened (P<0.01). But there was no significant difference between other two groups(P>0.05).Conclusions1. TRAIL and its receptorsDR4, DR5 involved in living-related small bowel transplant rejection.2.The low expression level of DR4, DR5 of INF-lymphocyte may have correlation with the occurrence of acute rejection.3.Reducing the down regulation expression level of DR4, DR5 might accelerate the apoptosis of INF-T-lymphocyte and restrain the occurance of actue rejection in graft jejunums.
