| Objective: To discuss the curative effect and possible mechanism of Vertebro -basilar Artery Insufficiency (VBI) treated with ShengyangLiqiao acupuncture therapy. We have conducted a randomized control study on 35 cases of VBI treated with cervical "jiaji" acupoint acupuncture. It was observed by research means of the change of main clinical symptom and physical signs, Transcranial Doppler(TCD), blood rheology, blood-lipid and fasting blood-glucose, et al. The theoretical evidence can be provided on VBI vertigo treated with acupuncture therapy.Methods: 70 cases of VBI were chosen and randomly divided into treatment group of 35 cases and control group of 35 cases in term of consulting random numeric table. The treatment group treated with ShengyangLiqiao acupuncture therapy and the control group treated with Cervical "jiaji"point acupumcture. The main acupoints selected were Baihui (GV20), Fengchi ( GB20 ) , Tianzhu (BL10) ,Wangu (GB12) , Dazhui (GV14) , Houxi (SI3) ,T aichong (LR3) , and the auxiliary acupoints were chosen according to differentiation of syndromes in treatment group. Cervical "jiaji" acupoint was chosen in control group treated with acupuncture method. The auxiliary points and manipulation were same like that in treatment group. It comprised 10 days in a course and treated once a day retaining needles for 30 minutes. They had been observed for two courses in total. Patients broke 2 - 3 days between two courses.The indexes were checked as bellows before the commencement and after the completion of two courses: (1) The change of main clinical symptom and physical sign of clinical curative effect. (2) Some indexes of blood flow velocity of systolic apex ( Vs), diastasis (Vd), mean ( Vm ) and artery pulsate index (PI) of vertebra-artery (VA) and basilar arterial (BA) checked by TCD. (3) The adhesion of the whole blood, plasma reduced degree, hematocrit and platelet adhesion reaction rate et al checked by blood rheology. (4) Blood-lipid including cholesterin(TC), triglyceride((TG), high density lipoprotein(HDL-C), and low density lipoprotein(LDL-C). ( 5 ) Fasting blood-glucose.Results:(1) The result manifests that it were healed 11 cases, effected 23 cases and inefficiency 1 cases ,the total effective rate was 97.1% in treatment group; and healed 4 cases, effected 24 cases, inefficiency 7 cases, the total effective rate was 80.0% in the control group .There is a significant difference between the two groups(P<0.01) .The curative effect in treatment group was better than that in control group.(2) There was a statistical meaning between the two groups (P<0.05) compared with clinical symptom and physical signs, Just before the commencement and after the completion of a course of treatment. The curative effect in treatment group was better than that in control group.(3) There was a statistical meaning on the indexes of blood rheology, blood — lipid and fasting blood—glucose between before the commencement and after the completion of two course in treatment group (P<0.05-P<0.01). However, only a pan of that changed in control group.(4) There were 9 cases showing quicker, 55 cases showing slower and 6 cases showing in gear in vertebro—basilar artery blood flow velocity checked by TCD.(5) The results checked by TCD showed that there wasn't statistical meaning between treatment and control group before the commencement on Vs, Vm , Vd and PI. But there was a significant difference compared with that before the commencement in treatment group itself and after the completion in control group(P<0.05). There was no difference between before the commencement after the completion in control group.Conclusion:Because the pathological mechanism of VBI is that yang qi doesn't rise, and blood stasis in Meridian, and seven orifices are insufficiency, and brain is failure nourished, the efficacy of ShengyangLiqiao acupuncture therapy is to promote blood circulation to remove obstruction from meridians and to nourish brain.
