In recent years, it has been commonly recognized that acute pancreatitis (AP), especially severe acute pancreatitis (SAP) is accompanied by intestinal endotoxemia (IETM). However, as for IETM produced by AP and AP onset mechanism, is there any correlations among the various theories, which include"Pancreatic enzyme autodigestion","Leucocyte excessive activation"and"Pancreas microcirculation disturbance". In addition, there is few reports concerning transformation mechanism and the relation between mild acute pancreatitis(MAP) and SAP.The researchers in the Liver Disease Institute of Shanxi Medical University, led by Prof. Han Dewu, have been working on the investigation of relationship between IETM and liver diseases in the past 20 years. They put forward a new opinion that IETM is produced when liver trauma is caused by various kinds of pathogenic factors (Primary hepatic injury) and the IETM can activate mononuclear macrophage system to release multi-cytokines, inflammatory medium and free radicals which would cause"secondary hepatic injury". It is obvious that IETM can be taken as the"second strike"to liver. The main research topic in my study is to explore whether the IETM accompanied with AP can make the"second strike"to pancreas; and whether anti-IETM medicines such as Shuangligan and Glycine are effective to the treatment of SAP.The study is divided into 4 parts:Part I---------The Dynamic Variety of IETM in Vivo and Its Correlation with CytokinePart II--------IETM & Pancreas Microcirculation DisturbancePart III------IETM & Organism Immunologic FunctionPart IV-----The Experimental Treatment of Anti-IETM Drug-- Shuangligan and Glycine on SAPPart I The Dynamic Variety of IETM in Vivo And Its Correlation with CytokineObjective: By studying the dynamic variety of endotoxin (ET) with SAP, it is revealed that there exists the relationship between ET and the severity degree of pathological changes with SAP and illustration of the correlation between ET and cytokine.Test 1 Dynamic variety of IETM in SAP developmentWe choose 40 Wistar rats weighing 260±20g with no limitation in their sexes. These rats are provided by Experimental Animal Center of Shanxi Medical University. They are divide into sham operation (SO) (n=8) and SAP model group (n=32) randomly.Method: Fast the rats without water deprivation for 12h before the operation and give...
