Evaluation Of Right Ventricular Function By Comprehensive Echocardiographic Techniques

As a main part of the circulation system the right ventricular plays an important role in maintaining the normal function of the circulation system. Up till now, there is still no a series study on the right heart by echocardiography domestically and abroad. In this study we combined two-dimensional, Doppler, three- and four-dimensional echocardiography, Transesophageal echocardiography , and the new technique-acoustic quantification with radionuclide ventriculography and right heart catheter examinations and measured the right ventricular function seriesly.We compared the right ventricular ejection fraction measured by three different two-dimensional calculations: Simpson's rule, single plane area-length, and the subtraction methods with RVEF by radionuclide ventriculography. The result showed that the three 2DE measurements correlated significantly with radionudide ventriculography(p<0.01 all). The coefficients of Simpson's, area-length, and subtract calculation were 0.97, 0.90, and 0.88 respectively. We also calculated the RV volumes and ejection fraction in 30 ASD patients and 20 age matched normal subjects, 25 pulmonary hypertensive patients and age matched 20 control subjects. The result showed that there was no difference of RVEF in ASD patients and its controls, but the RVEDV, RVESV. RVSV. RVSI, RVCO, and RVCI of ASD patients were all significantly higher than their control subjects. RVEF, RVCO, and RVCI of PH patients were all significantly lower than that of the normal subjects. But RVEDV, EVESV and RVSV were statistically higher than normal. We concludes that 2DE single plane area-length method is accurate, simple to use. It may be chosen to be a routine method for estimating RV volumes and EF.According to Newton's second law of motion(F=ma), we measured the right ventricular ejection force using Doppler combined with 2DE echocardiography in 35 patients underwent radionudide ventriculography and compared it with RVEF calculated by radionudide ventriculography.
