Part I: A Study of Neo-urethral MicroenvironmentObjective: The source of the bacterium in the neo-urethral is to be found. The most common floras that cause infection after hypospadias operation and the most sensitive antibiotic for hypospadias are to be reveled. The relation between neo-urethra wall and infection is to be made clear. The neo-urethral microenvironment theory (RUM theory) is to be primarily founded. Operation techniques and the perioperation disposal will be hereby improved so as to reduce the infection rate. Methods: The floras that cause infection was examined by germiculture and karyotype analysi. The infection related factors were found by reviewing and logistic regression analysis hypospadias cases histories. The inner wall of the neo-urethra and normal urethra were observed through urography and urethroscope. The tissue section of neo-urethra, normal urethra and the material for reconstruction urethra were observed through light microscope, scanning electron microscope and transmission electron microscope. Results: The bacteria in the neo-urethra come from urethra orifice and material for urethra reconstruction. The most common floras that cause infection are Gram-positive coccus.The most sensitive antibiotic for hypospadias is demethovancomycin. The infection related factors comprise age, different operation techniques, local condition, antibiotics used and different perioperation disposal. The tissue of the urethra and the material for urethra reconstruction are liable to infection in the same degree. Metaplasia never take place in neo-urethra. Conclusion: We put forward the following points in the world for the first time: the nature of infection is not infection of urinary system, but infection of incisions. The most common bacterium that cause the infection are not that cause infection of urinary system, but that cause skin infection. The bacterium in the neo-urethra come from urethra orifice and material for urethra reconstruction where they parasitize in the pilosebaceous ducts.Part II: Application of Neo-urethral Microenvironment Theory for Re-do Hypospadias Objective: The forming mechanism and therapying method of re-do hypospadias is to be discussed.
