Atherosclerosis(AS) is kind of pathological phenomenon which has deficient and excessive apoptosis. In the later stage of the AS, the apoptosis in the arteries become the predominant pathological process, especially in the AS plaque. The over apoptosis of AS plaque is unstable and can cause the acute cardiovascular and cerebrovascular diseases. This experiment ,using the thinking way of Traditional Chinese Medicine(TCM) from theory and experiment aspects, tried to make the systematic study and discussion in the field of the etiology, pathogenesis, therapeutic principles and herb prescription. Qi deficiency, phlegm, blood stasis and internal heat and toxin are considered as the main pathogenesis in AS, so that herb medicine prescription Shen Xia Decoction (Radix Ginseng and Rhizoma Pinelliae)with the therapeutic function of strengthening the Qi, dissolving phlegm, promoting blood circulation and relieving internal heat and toxin was made. Shen Xia Decoction's effect on the apoptosis of the cultured VSMC and AS plaque cell was observed. Methods: 1.It was established that the model of the apoptosis of the cultured rats VSMC induced by Angiotensin-â…¡(Ang-â…¡).The effects of Shen Xia Decoction on the apoptosis of VSMC and the expression of Caspase-3 were examined. 2.It were observed that the effects of Shen Xia Decoction on the apoptosis , the expression of Bax and Bcl-2 in the AS plaque of the aortic artery and the level of blood lipid of rats in vivo. Result: 1.The ratio of the apoptosis in cultured VSMC in lower dosage of Shen Xia Decoction group was significantly lower than the model group(P<0.05) and very obviously lower than Hong hua(Flos Carthami)Decoction control group(P<0.01). .The ratio of the apoptosis cultured VSMC in high dosage of Shen Xia Decoction group was very significantly lower than the model group and Hong hua(Flos Carthami)Decoction control group(P<0.01,P<0.01). 2.The expression levels of Caspase-3 in cultured VSMC in bothlower and high dosage of Shen Xia Decoction groups were obviously lower than the model group and Hong hua Decoction control group(P<0.05, P<0.05). 3.The typical AS pathological change in the rat aortic artery in the model group could be seen under microscope. In the AS plaque, there were a lot of cholesterol crystals. The pathological change in the rat aortic artery in the Shen Xia Decoction group was similar with the model group, however, the degree of seriousness of the pathological change was lighter than model group and there were not cholesterol crystals in the AS plaque. 4.The expression level of Bax in AS plaque in the rat aortic artery in Shen Xia Decoction group was significantly lower than the model group(P<0.05). There was not obvious difference between Shen Xia Decoction group and the model group in the expression level of Bcl-2 in AS plaque of rat aortic artery. 5.The ratio of apoptosis in AS plaque in the rat aortic artery in Shen Xia Decoction group was very significantly lower than the model group (P<0.01). 6.Compared with model group, Shen Xia Decoction could reduce the level of TG, TC and LDL-CH very significantly(P<0.01), very obviously raise the level of ApoA in the serum of experimental AS rats(P<0.01).Based on the experiment, this study suggests that Shen Xia Decocotion can inhibit the apoptosis of both cultured VSMC induced by Ang-â…¡and AS plaque and can reduce the level of blood lipid. The mechanism of Shen Xia Decoction with the function of reducing the apoptosis perhaps is caused by the inhibition of the expression of Caspase-3 and Bax which are important apoptosis transduction signals. Shen Xia Decoction maybe has important value of clinical application in the field of stabilizing the AS plaque and reducing the incidence of acute cardiovascular and cerebrovascular diseases.
