Clinical And Experimental Study Of Effect Of Continuous Blood Purification On Thrombotic And Fibrinolytic Factors And Mechanism

Background : Continuous blood purification (CBP) is a new effective method for the treatment of multiple organ dysfunction syndrome (MODS) .There are injuried in endothelial of patients with MODS. Thrombotic and fibrinolytic factors are secreted unusually from endothelial which result in disbalance between coagulation and anticoagulation.It shows increase of coagulation activity and decrease of anticoagulation.The coagulation system was activated which cause consumption of blood coagulation factor and hemorrhagic tendency,which develop to DIC.Blood get in touch with materials of extracorporeal circulation which activate thrombotic factors and coagulation cascade. CBP can clean up toxin and cytokine of patients which affect coagulation and fibronolysis. We find hypercoagulabale state was improved in most patients with MODS by CBP.But hemorrhagic complication are common among CBP patients even if without anticoagulation.Vascular endothelial cells(VEC) participate actively adjustment of coagulation and fibronolysis.VEC can secrete thrombotic and fibrinolytic factors,which play important role on coagulation and fibronolytic system during CBP.The study is very few about CBP affecting vacular endothelial cells and thrombotic and fibrinolytic factors secreted from VEC. Our study investigate the rule of thrombotic factors such as tissue factor,tissue factor pathway inhibitor,plasminogen activator inhibitor-1,and the rule of cytokine such as tumor necrosis factors-α, interleukin-1β,and the interrelationship among them.In addition,we observe the effect of serum from patients with MODS during CBP on thrombotic and fibrinolytic factors secreted from vascular endothelial cells,and the effect of heparin on them.The aim of this study is identify the effect of CBP on thrombotic and fibrinolytic factors and its mechanism.Objective:To explore the effect of continuous blood purification on thrombotic and fibrinolytic factors and its mechanism under the...
