Studies Of Preoperative Epileptogenic Focus Localization In Refractory Temporal Lobe Epilepsy And Association Between Interleukin-1β Gene Polymorphism And Hippocampal Sclerosis In Chinese Population

Background and Objectives: There are at least 5 million epileptics in China, of which 20-25% patients have refractory seizures. Among the patients with intractable seizures, about 15% of patients may benefit from surgery. Given that there has been increasing interest in the surgical treatment of patients with intractable seizures in China, it's necessary to have correct evaluation for the values of preoperative diagnostic modalities.In this study, we attempted to assess the value of electrophysiological(EEG),structural and functional neuroimaging neuroimaging (MRI/SPECT/PET) techniques in presurgical evaluation for medically intractable nonlesional temporal lobe epilepsy(TLE). Factors affecting on seizure outcome were also analyzed. The most common pathology in adult patients with nonlesional TLE is hippocampal sclerosis(HS) ,but pathological origin of HS still remains unknown. There is a controversy on the relationship between a single nucleotide polymorphism (SNP)in Interleukin-1β gene promoter (IL-1β-511) and genetic susceptibility to TLE with hippocampal sclerosis (TLE-HS~+).In this study,we alao try to investigate the relationship in Chianese population.Methods:1. The data of presurgical diagnostic modalities and postsurgical follow-up from 18 Patients (male: 11 cases;female:7cases;av.28.9yr.)with intractable non-lesional TLE were analyzed.Presurgical investigations include interictal/ictal scalp Video-EEG,high-resolution MRI, interictal/ictal SPECT, interictal PET and Wada test.Relationship between different abnormality patterns in each modality and surgical outcome were evaluated. Factors affecting on seizure outcome were analyzed.We also compared the the efficacy for localization of epileptogenic foci among modalities.
