The first and second branchial arch syndrome is the moet common congenital craniofacial deformity besides the lip/palat c!eft.Recently the cognizanceand treatment is advance slowly for the first and second branchial arch syndrome. We summed up a great amount of the literature on the cognizance and treatment of the syndrome. We have done the cephalometric analysis of the syndrome for 16 children and corrected early mandibular deformity using mandible lengrhening for 12 children.Part I, Reference review: We have studied the developmental history of the cognizance and treatment of the first and second branchial arch syndrome. It was emphasized to review themandible maldevelopment-------key deformity among the syndrome,which may trigger a "domino effect" , and early treament in order to stop the "domino effect" .Part II, Experiment study: According to the blood aupply to mandible we designed the osteotomy menthod in mandible lengthening in order to protect the inferior alveolar nerve vasculature.
